血行播散型肺结核并发呼吸衰竭患者行人工机械通气治疗的护理

摘　　要：2018年11月20日笔者所在科收治1例血行播散型肺结核并发呼吸衰竭患者行人工机械通气治疗,历经17 d,通过医护紧密协作,对患者尽早纠正呼衰,控制感染因素,早期采用人工气道机械通气,联合使用镇静剂和肌松剂抑制呼吸,配合床边纤维支气管镜检查,有效排除分泌物,改善通气,提高其生存率。同时通过护理人员做好病情评估,制定护理计划,落实密切病情观察,做好人工气道管理,基础护理,给予心理支持等各项护理措施,严格执行操作规程,做好消毒隔离及健康指导后,患者于2018年12月7日办理出院。连续随访5个月后,患者病情稳定,未再复发。On November 20 th,2018,1 case with blood disseminated type pulmonary tuberculosis complicated with respiratory failure was treated by artificial mechanical ventilation in our department.After 17 days,through close cooperation of medical care,the patients were corrected for respiratory failure as soon as possible,and infection factors were controlled.Artificial airway mechanical ventilation was used in the early stage,sedatives and muscle relaxants were used to inhibit respiration.Cooperate with bedside fiberoptic bronchoscopy,effectively eliminate essential substances,improve ventilation and increase the survival rate.At the same time,through nursing staff to do a detailed assessment of the condition,make a nursing plan,implement close observation of the condition,make good management of artificial airway,basic nursing,provide psychological support and other nursing measures,strictly implement the operating procedures.After disinfection,isolation and health guidance,the patient was discharged on December 7 th,2018.After 5 months of continuous follow-up,the patient’s condition was stable and did not relapse.
