摘 要:目的:探究社区获得性下呼吸道感染患者病原菌及其耐药性变迁情况。方法:选取2016年10月-2018年10月笔者所在医院收治的社区获得性下呼吸道感染患者200例,仔细检测所有患者的痰标本,分析研究其病原菌分布及其耐药性情况。结果:200例患者中检测出180株病原菌,检出率为90.00%,其中革兰阳性菌、革兰阴性菌和真菌所占比率分别为33.33%(60/180)、58.33%(105/180)、8.33%(15/180)。表皮葡萄球菌、肺炎链球菌及金黄色葡萄球菌对甲氧苄啶、红霉素及青霉素有较高耐药率;大肠埃希菌、铜绿假单胞菌及肺炎克雷伯菌对头孢呋辛、哌拉西林及氨苄西林有较高耐药率。结论:革兰阴性菌是社区获得性下呼吸道感染患者的主要病原菌,同时存在十分突出的耐药性,因此在用药和诊断期间需要加强重视度。Objective:To investigate the pathogens and drug resistance changes of patients with community acquired lower respiratory tract infection.Method:A total of 200 patients with community-acquired lower respiratory tract infection admitted to our hospital from October 2016 to October 2018 were enrolled.All patients were examined for sputum samples and their pathogen distribution and drug resistance were analyzed.Result:Among the 200 patients,180 pathogens were detected,accounting for 90.00%.The ratio of Gram-positive bacteria,Gram-negative bacteria and fungi was 33.33%(60/180),58.33%(105/180)and 8.33%(15/180),respectively.Staphylococcus epidermidis,Streptococcus pneumoniae and Staphylococcus aureus were highly resistant to Trimethoprim,Erythromycin and Penicillium.Escherichia coli,Pseudomonas aeruginosa and Klebsiella pneumoniae were highly resistance to Cefuroxime,Piperacillin and Ampicillin.Conclusion:Gram-negative bacteria are the main pathogens of patients with community-acquired lower respiratory tract infections,and there is a strong drug resistance.Therefore,it is necessary to strengthen the importance during medication and diagnosis.
