多层螺旋CT在儿童卵巢蒂扭转中的诊断价值

摘　　要：目的总结儿童卵巢蒂扭转的多层螺旋CT图像特征,探讨其影像诊断价值。方法回顾性分析我院2013年1月~2017年6月经手术病理证实的20例儿童卵巢扭转患者的临床资料及CT图像特点。术前均采用多层螺旋CT平扫,11例行多期增强扫描。结果本组20例患者,右侧14例,左侧6例,原发性卵巢蒂扭转7例,卵巢畸胎瘤合并扭转10例,卵巢囊肿合并扭转3例。多层螺旋CT平扫表现为盆腔内囊实性、囊性或混杂密度肿块。20例中,患侧卵巢均肿大,密度增高,13例增大卵巢表面可见数量不等的增大滤泡,呈蜂窝状改变。8例可显示卵巢扭转蒂。13例子宫不同程度向患侧卵巢移位。14例盆腔内可见少量积液。11例CT增强患者中,卵巢包膜均可见强化,包膜下低密度影未见强化,9例卵巢实质轻度强化,2例不强化。患侧卵巢周围血管增粗、走行迂曲。结论儿童卵巢扭转的多层螺旋CT表现具有一定的特征,多层螺旋CT平扫加增强检查在儿童卵巢扭转中具有重要辅助诊断价值,能为临床治疗提供参考依据。Objective To summarize the MSCT evaluation of ovarian pedicle torsion of children,and to explore the value of imaging diagnosis.Methods The CT data and clinical information of 20 cases of ovarian torsion were retrospectively analyzed between January 2013 to June 2017.Before operation,all patients were examined with spiral CT,11 cases with multiphase enhancement scans.Results All the 20 cases,14 cases were on the right,6 cases on the left.There were 7 cases of primary ovarian torsion,10 cases of ovarian teratoma combined with torsion,3 cases of ovarian cyst combined with torsion.In the multilayer spiral CT,the surface scan appeared to be a vesicular,cystic,or mixed density mass in the pelvic cavity.In 20 cases,the ovary was swollen,the density increased,and 13 cases increased the number of enlarged follicles on the surface of the ovary,showing Honeycomb changes.The uterus shifted to the side ovary to different degrees.8 cases of ovarian pedicle torsion were seen.In 13 cases,the uterus shifted to the side ovary to different degrees.A small amount of effusion was observed in 14 cases of pelvic cavity.In 11 cases of enhancement,ovary envelope showed enhancement,low density shadow under the envelope were not enhanced,9 cases presented with mild enhancement of ovarian parenchyma,2 cases were not enhanced.The blood vessels around the side of the ovary were thickened and walking.Conclusion The children ovarian pedicle torsion has certain characteristics on MSCT,CT enhanced scanning is of great value in the diagnosis of ovarian torsion in children.It can understand torsion syntheses and provide reference and basis for clinical treatment.
