多模式超声成像在兔股静脉血栓分期诊断中的初步研究

摘  要:目的探讨多模式超声成像在兔股静脉血栓分期诊断中的价值。方法建立兔股静脉血栓模型,利用灰阶超声观测术区股静脉管径和回声变化,超声剪切波弹性成像(ultrasound shear wave elastography,SWE)技术检测血栓体部的杨氏模量值,并在第3、9、16天检查结束后随机取6只兔子制备血栓病理切片。结果病理切片可见1~16天血栓经历了从急性期到慢性期的转变。反应血栓直径变化的股静脉管径值随时间延长表现出先增大后稍变小的阶段性特点。第3、6、10天之间的管径值差异有统计学意义,P<0.05,说明血栓直径变化在急性亚急性期之间差异有统计学意义,亚急性慢性期差异不明显,第1~9天的血栓的回声无明显变化,第9~16天的灰度值明显增加,由均匀低回声向不均匀等高回声转变。SWE测量结果显示血栓的杨氏模量值随时间延长呈不同速率的增加,第1、3、8、16天之间的杨氏模量值有统计学意义,P<0.05,第9、16天比较差异无统计学意义,P>0.05,但第9、16天两者回声及与周边组织分界差异性显著,且病理切片证实两者分别属于亚急性、慢性期。结论多模式超声成像联合诊断提高了兔股静脉血栓分期的准确性。Objective To explore the value of multi-mode ultrasound imaging in the staging diagnosis of femoral venous thrombosis in rabbits.Methods The model of rabbit femoral vein thrombosis was established,gray-scale sonography was used to observe the changes in femoral vein diameter and echo in the operative area.Ultrasound shear wave elastography(SWE)technology was used to detect the young's modulus value of the thrombus body.At the end of day 3,9 and 16,6 rabbits were randomly selected to prepare thrombus pathological sections.Results Pathological sections showed that the thrombus underwent a transformation from the acute phase to the chronic phase at 1~16 days.The diameter of femoral vein which reflected the change of thrombus diameter first increased and then decreased.Pipe diameter value between day 3,6,and 10 was statisticant differences,P<0.05,suggesting that thrombus diameter change difference was statisticant differences between acute and subacute stage,it was not obvious between subacute and chronic phase,from day 1 to day 9,the echo of the thrombosis had no obvious change,from day 9 to day 16,the grey value increased significantly,from the homogeneous low echo to uneven medium-high echo.SWE measurement results showed that the young's modulus value of thrombosis increased in different rate with the extension of time,young's modulus value in day 1,3,8 and 16 was statisticant differences,P<0.05,there was no statistically significant difference in day 9 and 16,P>0.05,but the echo and the boundary with surrounding tissue were different in day 9 and day 16 that subacute and chronic pathologically confirmed respectively.Conclusion Multi-mode ultrasound imaging combined with diagnosis can improve the accuracy of femoral venous thrombosis staging in rabbits.
