腔道表麻下输尿管镜处理输尿管末端（下段）结石的手术治疗

摘　　要：目的探讨腔道表麻下输尿管镜处理输尿管末端(下段)结石的手术治疗。方法选取2017年6月~2019年5月我院收治的48例采用输尿管镜处理输尿管末端(下段)结石患者作为研究对象,随机分为观察组与对照组,每组各24例。观察组采用腔道表面麻醉,对照组采用硬腰联合麻醉。比较两组的手术时间、视觉模拟量表(VAS)评分、碎石成功率、住院天数、住院费用及并发症发生情况。结果观察组的手术时间短于对照组,差异有统计学意义(P<0.05)。两组的VAS评分、碎石成功率比较,差异无统计学意义(P>0.05)。观察组的住院天数短于对照组,住院费用低于对照组,差异有统计学意义(P<0.05)。观察组的并发症总发生率低于对照组,差异有统计学意义(P<0.05)。结论腔道表面麻醉下输尿管镜处理输尿管末端(下段)结石可缩短手术时间与住院天数,减轻患者的经济负担,且可降低麻醉并发症发生率,是一种安全、有效、经济的麻醉方式,值得在临床上推广应用。Objective To investigate the surgical treatment of ureteral terminal(lower segment)calculi with ureteroscopy under the surface anesthesia of the lumen.Methods A total of 48 patients who underwent ureteroscopy for ureteral terminal(lower segment)calculi from June 2017 to May 2019 in our hospital were enrolled as subjects,they were randomly divided into the observation group and control group,with 24 cases in each group.In the observation group,surface anesthesia of the lumen was adopted,while in the control group,combined spinal-epidural anesthesia was used.The operation time,visual analogue scale(VAS)score,lithotripsy success rate,length of hospitalization,hospitalization cost and complications were compared between the two groups.Results The operation time of the observation group was shorter than that of the control group,the difference was statistically significant(P<0.05).There were no significant differences in VAS score and gravel success rate between the two groups(P>0.05).The length of hospitalization in the observation group was shorter than that in the control group,and the hospitalization cost was lower than that in the control group,with statistically significant differences(P<0.05).The total complication rate in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).Conclusion Ureteroscopy under the surface anesthesia of the lumen for ureteral terminal(lower segment)calculi can shorten the operation time and length of hospitalization,reduce the economic burden of patients,and decrease the rate of anesthesia complications.It is a safe,effective and economical anesthetization,and is worthy of promotion in clinical practice.
