摘  要:目的探讨成骨细胞复合富血小板血浆(plateletrich plasma,PRP)凝胶移植对兔桡骨牵张成骨的作用。方法64只成年家兔随机分为A、B、C、D组,每组16只,于双侧桡骨中上段安装骨延长架,并行横断截骨,7 d后开始行骨牵引手术,每天延长2次,每次延长0.5 mm,牵张2周。延长结束后A组不作处理,B组在骨延长区局部注射成骨细胞凝胶500μL,C组在骨延长区局部注射富血小板血浆500μL,D组在骨延长区局部注射成骨细胞复合富血小板血浆500μL,分别在骨延长术后1、2和3周注射,术后4周将全部实验动物处死行X线检查,同时对骨延长区骨组织进行组织学切片,HE染色观察新骨生成情况。结果术后4周,X线结果显示:A组延长段可见骨折线,B、C组延长段完全被骨质取代,D组骨痂密度较高,骨皮质已完全连续;术后4周组织学观察A组局部区域尚有残存的软骨;B、C组骨小梁内胶原纤维增多,D组新生骨区域光新生骨开始改建,兔骨折断端X线和组织学评分B、C、D组均高于A组(P<0.05或<0.01);D组高于B和C组(P<0.05)。结论成骨细胞复合富血小板血浆对牵张成骨中新骨生成具有显著促进作用,可以加快牵引成骨过程的新骨生成。Objective To discuss the effect of osteogenic cells combined with platelet rich plasma gel on rabbit bone distraction osteogenesis.Methods Total 64 adult rabbits were randomly divided into A,B,C,D groups,with 16 rabbits in each group.Bone lengthening frame was installed in the middle and upper part of the radius,and osteotomy was performed in parallel.Seven days later,bone traction operation was started,extending twice a day,extending 0.5 mm each time,stretching for 2 weeks.After the end of the extending,the rabbits in group A were not treated.In group B,the osteoblasts were injected with 500μL osteoblast gel in the bone extension area.The platelet rich plasma was injected into the bone lengthening area of the rabbits in group C for 500μL.The D group was injected with osteoblasts and platelet rich plasma 500μL in the bone extension area.One week,2 weeks and 3 weeks after bone lengthening,all the experimental animals were sacrificed for X-ray examination,and the bone lengthening was performed at the same time.The new bone formation was observed by HE staining.Results Four weeks after the operation,X-ray results showed that fracture lines of the extension segment could be seen in group A.In group B and C,the extension segment of was completely replaced by bone.In group D callus density was relatively high and bone cortex was completely continuous as well.Four weeks after the operation,histological observation results showed that residual cartilage was in the local area in group A.The collagen fibers in the bone trabeculae increased in group B and C.The Lane-Sandhu radiographic and histological scoring showed that scores in group B,C and D were significantly higher than those in group A at the 4 th week after surgery(P<0.05 or<0.01),which was the highest in group D.Conclusion Osteoblast combined with platelet-rich plasma can significantly promote the formation of new bone in distraction osteogenesis.It can accelerate the formation of new bone during distraction osteogenesis.
