50例类风湿关节炎患者HLA-DR4基因检测及临床指标分析

摘　　要：目的了解宁夏地区类风湿关节炎患者HLA-DR4基因分布频率,以及类风湿关节炎临床指标。方法选取宁夏医科大学总医院收治的宁夏地区类风湿关节炎患者50例为病例组,同时期本院体检健康者50例为正常对照组,采用序列特异性引物-聚合酶链式反应(PCR-SSP)检测血液HLA-DR4基因分布频率,并检测血细胞[红细胞(WBC)、白细胞(RBC)、血小板(PLT)]计数、血红蛋白(HGB)、免疫球蛋白(IgG、IgA、IgM)、补体(C3、C4)、炎性指标[血沉(ESR)、超敏C反应蛋白(CRP)]以及双手X线片等相关实验室指标。结果宁夏地区50例类风湿关节炎患者中,HLA-DR4阳性患者26例,阴性患者24例,HLA-DR4表达频率(52%)高于正常对照组(14%),HLA-DR4基因阳性类风湿关节炎患者较阴性患者病程长(P<0.05)。结论 HLA-DR4可能为宁夏地区类风湿关节炎患者易感基因,HLA-DR4阳性类风湿关节炎患者病程更长。Objective To explore the frequency of HLA-DR4 gene and serological characteristics of patients with rheumatoid arthritis in Ningxia population. Methods Fifty rheumatoid arthritis patients and 50 matched healthy control subjects were examined separately with polymerase chain reaction-sequence specific primer(PCR-SSP)to get the gene frequencies. Results Among 50 patients with rheumatoid arthritis in Ningxia,26 patients were positive for HLA-DR4 and 24 were negative. The expression frequency of HLA-DR4 was 52%,which wassignificantlyhigher than thatofthenormalcontrolgroup(14%).The disease course of HLA-DR4-positive patients showed longer than HLA-DR4 negative patients,and there was no significant difference between the HLA-DR4 positive and HLA-DR4 negative patients with respect to serological characteristics,X-ray of hands and disease activity(P<0.05).Conclusion HLA-DR4 may be a risk factor for rheumatoid arthritis in Ningxia.HLA-DR4 is strongly associated with disease course in rheumatoid arthritis.
