917例女性生殖道尖锐湿疣组织HPV感染基因分型的研究

摘  要:目的低危型人乳头瘤病毒(low-risk human papillomavirus,LRHPV)感染是女性生殖道尖锐湿疣(condyloma acuminata,CA)发生的主要病因。文中旨在探讨以江苏省为主的区域性女性生殖道尖锐湿疣组织中人乳头瘤病毒感染基因型分布状况和分型的临床意义。方法收集来自江苏省和安徽省在内的8家医院917例女性生殖道尖锐湿疣组织标本,从其尖锐湿疣组织中抽提HPV DNA,使用基因芯片结合聚合酶链式反应技术对所有尖锐湿疣组织检测23种HPV;并以10岁为极差,对受检者HPV感染型别分布状况进行分析。结果917例女性生殖道尖锐湿疣患者检出HPV感染者805例,总的HPV感染率为87.79%(805/917),其中一型感染率为55.07%(505/917),以11、6和16型最为常见,分别占一型感染51.47%(472/917)、38.71%(355/917)和2.84%(26/917)。多型感染率为32.72%(300/917),以6+11型、11+18型、11+16型和6+16型为主,其分别占多型感染12.00%(36/300)、8.67%(26/300)、5.00%(15/300)和4.33%(13/300)。结论江苏省为主的区域性女性生殖道尖锐湿疣组织中HPV感染,以一型感染为主,多型感染为辅,11、6和16是最为常见的感染型别,行生殖道HPV分型检测,这对我国女性生殖道尖锐湿疣和宫颈病变的防治都具有十分重要的意义。Objective To explore the clinical distribution states of 23 kinds of human papillomavirus genetypes in tissues of 917 women with reproductive canal condyloma acuminates in Anhui Province and Jiangsu Province and genotyping clinical significance.Methods We collected tussue amples of reproductive canal condyloma acuminates from 917 women in 8 hospitals of Anhui Province and Jiangsu Province.We extracted HPV DNA from tussues of vulval、vaginal and cervical condyloma acuminates,detected 23 of all the HPV genetypes using PCR and gene-chips,and analyzed the distribution of the HPV infection types among the subjects of different age groups.Results In 917 women of reproductive canal condyloma acuminates,805 women of HPV infecton,total infection rate of HPV was 87.79%(805/917),including single genotype infection rate of HPV was 55.07%(505/917),11、6 and 16 genotypes are the most common in single genotypes,they are successively 51.47%(472/917)、38.71%(355/917)and 2.84%(26/917).multiple genotypes infection rate of HPV was 32.72%(300/917),6+11、11+18、11+16 and 6+16 genotypes are the most common in multiple genotypes,they are successively 12.00%(36/300)、8.67%(26/300)、5.00%(15/300)and 4.33%(13/300).Conclusions Single genotype was primary way of HPV infection,multiple genotypes were secondary way of HPV infection in tissues of in tissues of 917 women with reproductive canal condyloma acuminates in Anhui Province and Jiangsu Province.11、6 and 16 types are most common infection types,The reproductive canal examine of HPV types should be held to the reproductive canal condyloma acuminates patients,This precaution will has extremely important meaning to the prevention and treatment of the female reproductive canal condyloma acuminates and cervical lesion in our nation.
