个性化颈过伸体位对甲状腺手术患者椎动脉血流动力学的影响

摘　　要：目的探究个性化颈过伸体位对甲状腺手术患者椎动脉血流动力学的影响。方法选取2016年12月~2018年12月南京大学医学院附属鼓楼医院住院的甲状腺手术患者112例,按照随机数表法分为对照组和观察组,每组各56例。对照组给予常规手术体位,观察组给予个性化颈过伸体位。比较两组术后甲状腺手术体位综合征的发生率、不同颈过伸位双侧椎动脉血流量变化及临床满意度。结果术后1周,观察组甲状腺手术体位综合征的发生率低于对照组;不同颈过伸位双侧椎动脉血流量变化低于对照组;临床满意度高于对照组;差异均有统计学意义(均P<0.05)。结论个性化颈过伸体位可降低甲状腺手术体位综合征的发生率,改善不同颈过伸位双侧椎动脉血流量变化,并提升满意度。Objective To explore the effect of personalized hyperextension of neck posture on vertebral artery hemodynamics in patients undergoing thyroid surgery.Methods From December 2016 to December 2018,112 cases with thyroid surgery admitted to Nanjing Drum Tower Hospital,the Affiliated Hospital of Nanjing University Medicine School were divided.According to the random number table method,the patients were divided into the control group and the observation group,with 56 cases in each group.The control group was given routine thyroid surgery posture,while the observation group was given personalized hyperextension of neck posture.The incidence of postural syndrome,changes of bilateral vertebral artery blood flow in different carotid hyperextension positions and clinical satisfaction were compared.Results One week after operation,the incidence of posture syndrome of the observation group was lower than that of the control group,and the changes of bilateral vertebral artery of blood flow of the observation group was lower than that of the control group,while the clinical satisfaction of the observation group was higher than that of the control group,and the differences were statistically significant(all P<0.05).Conclusion Personalized hyperextension of neck posture can reduce the incidence of thyroid surgery position syndrome,improve changes of bilateral vertebral arteries blood flow,and improve satisfaction.
