陷胸逐瘀汤治疗痰瘀毒互结型冠心病心绞痛疗效及对心肌耗氧量的影响

摘　　要：目的研究陷胸逐瘀汤治疗痰瘀毒互结型冠心病心绞痛患者临床疗效及对心肌耗氧量的影响。方法将2017年12月—2018年11月在陕西省中医医院门诊及住院治疗80例痰瘀毒互结型冠心病心绞痛患者随机分为2组,每组40例,对照组给予常规冠心病心绞痛治疗,观察组在对照组治疗基础上给予自拟陷胸逐瘀汤口服。观察2组患者治疗4周后心绞痛发作情况、硝酸甘油含服用量、心肌耗氧量及中医症状积分、西雅图心绞痛评分量表(SAQ)评分,比较2组临床疗效及中医症状疗效。结果治疗4周后,2组心绞痛发作次数明显减少且持续时间缩短,硝酸甘油含服用量减少,心肌耗氧量降低,中医症状积分明显降低,SAQ评分量表各项评分均明显提高,且观察组各指标改善情况均明显优于对照组,差异有统计学意义(P均<0.05)。观察组临床疗效总有效率及中医症状总有效率分别为92.5%(37/40)和95.0%(38/40),对照组分别为75.0%(30/40)和67.5%(27/40),观察组均高于对照组(P均<0.05)。结论陷胸逐瘀汤治疗痰瘀毒互结型冠心病心绞痛疗效显著,可有效改善患者中医症状及临床症状,减少心肌耗氧,提高生活质量。Objective It is to study the clinical efficacy of Xianxiong Zhuyu decoction in the patients with coronary heart disease with angina pectoris of type of phlegm,blood stasis and toxin binding and its effect on myocardial oxygen consumption.Methods Eighty outpatients and inpatients with coronary heart disease angina pectoris(phlegm,blood stasis and poison syndrome)treated in Shaanxi Provincial Hospital of Traditional Chinese Medicine were randomly divided into two groups,40 patients in each group.The control group was given conventional therapy for coronary heart disease and angina pectoris,and the observation group was given the self-prepared Xianxiong Zhuyu decoction based on the control group.The incidence of angina pectoris,dosage of sublingual nitroglycerin,myocardial oxygen consumption,scores of TCM symptoms and Seattle angina questionnaire(SAQ)were observed in the two groups after 4 weeks of treatment.The clinical efficacy and TCM syndrome efficacy were compared between the two groups.Results After treatment,the incidence of angina pectoris was significantly decreased,and the duration was shortened,the dosage of sublingual nitroglycerin,myocardial oxygen consumption and scores of TCM symptoms were significantly decreased,the scores of every item in SAQ were significantly increased,and the improvements of these indexes in the observation group were better than those in the control group,all the differences were significant(P<0.05).The total effective rate of the clinical efficacy and TCM syndrome efficacy in the observation were 82.5%(37/40),95.0%(38/40)respectively,and those in the control group were 75.0%(30/40)and 67.5%(27/40)respectively,the total effective rates in the observation group were higher than those in the control group(P<0.05).Conclusion Xianxiong Zhuyu decoction which can effectively improve TCM syndrome and clinical symptoms,reduce myocardial oxygen consumption and improve quality of life is effective in the treatment of coronary heart disease with angina pectoris of type of phlegm,blood stas
