茵陈加味汤联合基础西药治疗妊娠期肝内胆汁淤积症的临床疗效

摘　　要：目的:探讨茵陈加味汤联合基础西药治疗妊娠期肝内胆汁淤积症的临床疗效。方法:选取2016年1月—2019年2月医院收治150例妊娠期肝内胆汁淤积症患者临床资料,采用随机数字表达法将所有妊娠期肝内胆汁淤积症患者分为治疗组与对照组,每组各75例。对照组采用基础西药治疗,治疗组采用茵陈加味汤联合基础西药治疗。比较两组患者的疗效、分析两组患者的肝功能指标水平、分娩结局、产后并发症和新生儿结局情况。结果:治疗组总有效率明显高于对照组(86.67%>72.00%)(P<0.05)。治疗后,两组患者ALT、AST、TBIL、TBA、DBIL水平和瘙痒评分均比治疗前均降低,且治疗组ALT、AST、TBIL、TBA、DBIL水平和瘙痒评分降低幅度比对照组大(P<0.05)。治疗组瘙痒消失时间和黄疸消失时间较对照组缩短(P<0.05)。治疗组分娩结局、产后并发症和新生儿结局均显著优于对照组(P<0.05)。结论:茵陈加味汤联合基础西药治疗妊娠期肝内胆汁淤积症,可提高疗效,改善肝功能指标水平、分娩结局、产后并发症和新生儿结局。Objective:To investigate the effect of Modified Yinchen Decoctionand basic western medicine on intrahepatic cholestasis of pregnancy.Method:This prospectivestudy had a total of 150 patients with intrahepatic cholestasis of pregnancy admitted in our hospital from January 2016 to February 2019.The patients were arranged in order according to their order of admission,and were divided into treatment group and control group by random number table method.Seventy-five cases were in each group.Control group was treated with basic western medicine.Treatment group was treated with Modified Yinchen Decoctionand basic western medicine.After treatment,the clinical efficacy,the levels of the biochemical indicators,and birth outcome,postpartum complications and neonatal outcomes were compared in two groups.Result:The total effective rate in treatment group was higher than in the control group(86.67%>72.00%)(P<0.05).After treatment,the levels of ALT,AST,TBIL,TBA,DBIL and the pruritus score in two groups were lower than those before treatment(P<0.05).The levels of ALT,AST,TBIL,TBA,DBIL and the pruritus score in the treatment group were lower than those in the control group after treatment(P<0.05).After treatment,the itch disappearing time and jaundice disappearing time in treatment group were lower than those in the control group(P<0.05).In treatment group,birth outcome,postpartum complications and neonatal outcomes were all signifcantly better than those in the control group(P<0.05).Conclusion:Modified Yinchen Decoctionand basic western medicinecan improve the clinical efficacy of intrahepatic cholestasis of pregnancy,and improve birth outcome,postpartum complications and neonatal outcomes in intrahepatic cholestasis of pregnancy.
