摘  要:目的探讨中药二参汤对心肌梗死后心力衰竭大鼠AngII通路表达及炎性因子产生的影响及分子机制。方法将160只雄性Wistar大鼠,采用随机分组法,分为假手术组(10只)和模型组(150只),经冠脉结扎术诱导慢性心力衰竭造模成功后筛选出30只心力衰竭大鼠,进一步随机区组分为缬沙坦组、中药组、空白模型组,用西药缬沙坦做为阳性对照药组。观察二参汤对心肌梗死后心力衰竭大鼠B型脑钠肽、血管张紧素II、核转录因子-κB、C反应蛋白等相关炎症因子表达的影响,同时提取心肌组织进行切片,并使用Masson在光镜下观察其心肌的损伤程度。结果与CHF模型组相比,各给药组CHF大鼠CRP水平均明显下降,差异有统计学意义(P<0.05);CHF模型组大鼠心肌Ang II、NF-κB蛋白表达较假手术组明显升高;而各给药组Ang II、NF-κB蛋白表达明显低于CHF空白模型组;缬沙坦组和中药组CHF大鼠心肌组织的Masson染色显示其心肌损伤均有不同程度的减轻。结论二参汤可以调节BNP的过度表达,改善CHF大鼠的炎症状态,从而降低CHF大鼠心肌和血浆中的CRP含量,抑制心肌组织Ang II、NF-κB炎症因子表达。Objective To investigate the effect of Ershen Decoction on the expression of AngII pathway and the production of inflammatory factors in rats with heart failure after myocardial infarction and its molecular mechanism. Methods 160 male Wistar rats were randomly divided into two groups, with 10 rats in the sham operation group and 150 rats in the model group. 30 rats with heart failure were selected after successful establishment of chronic heart failure model induced by coronary artery ligature which were further randomly divided into valsartan group, traditional Chinese medicine group and blank model group. The Western medicine valsartan was used as the positive control group. The effect of Ershen Decoction on the expression of type B brain natriuretic peptide, angiotensin II, nuclear factor-κB and C-reactive protein in rats with post-infarction heart failure were observed. At the same time, the myocardial tissue was extracted, and the degree of myocardial injury was observed under the light microscope by Masson. Results Compared with the CHF model group, the CRP level of CHF rats in each administration group was significantly lower than that in the sham operation group (P<0.05), while the expression of Ang II, NF-κB protein in each administration group was significantly lower than that in the CHF blank model group, and the Masson staining in the myocardial tissue of CHF rats in valsartan group and traditional Chinese medicine group showed that the myocardial injury was alleviated in varying degrees. Conclusion Ershen Decoction can regulate the overexpression of BNP, improve the inflammatory state of CHF rats, decrease the content of CRP in myocardium and plasma of CHF rats, and inhibit the expression of Ang II, NF-κB inflammatory factor in myocardial tissue.
