吡柔比星、长春新碱、环磷酰胺、泼尼松联合治疗复发与难治性急性淋巴细胞白血病的效果

摘　　要：目的探讨吡柔比星、长春新碱、环磷酰胺、泼尼松(VTCP方案)联合治疗复发与难治性急性淋巴细胞白血病(ALL)的效果。方法选取2015年2月至2018年6月焦作市第二人民医院14例复发与难治性急性淋巴细胞白血病患者为研究对象,14例复发与难治性急性淋巴细胞白血病患者均接受VTCP方案治疗,记录其疗效、治疗前后实验室指标变化情况、治疗期间毒副反应发生情况。结果 14例复发与难治性ALL患者经相应方案治疗后总有效率为78.57%(11/14);治疗期间14例患者存在不同程度的恶心呕吐、脱发、骨髓抑制等毒副反应,未见心肝肾损害;14例患者治疗后白细胞计数、幼稚细胞比例显著低于治疗前,治疗后血红蛋白、血小板计数显著高于治疗前(P<0.05)。结论应用吡柔比星+长春新碱+环磷酰胺+泼尼松联合方案对复发与难治性ALL患者治疗有效性、安全性均较优。Objective To investigate the effect of pirarubicin, vincristine, cyclophosphamide and prednisone combined treatment on relapsed and refractory acute lymphoblastic leukemia(ALL). Methods Fourteen patients with relapsed and refractory acute lymphoblastic leukemia in the Second People’s Hospital of Jiaozuo from February 2015 to June 2018 were selected as the study subjects. Fourteen patients with relapsed and refractory acute lymphoblastic leukemia were treated with VTCP regimen. The efficacy, laboratory indicators changes before and after treatment were recorded, occurrence of toxic and side effects during treatment.Results The total effective rate of 14 patients with relapsed and refractory ALL was 78.57%(11/14). During the treatment period, 14 patients had different degrees of nausea and vomiting, alopecia, bone marrow suppression and other toxic side effects, and no damage to heart, liver and kidney. After treatment, the white blood cell count and the proportion of immature cells in 14 patients were significantly lower than before treatment, and after treatment, the hemoglobin level and platelet counts were significantly higher than those before treatment(P<0.05). Conclusions The combination of pirarubicin, vincristine, cyclophosphamide and prednisone is effective and safe in the treatment of relapsed and refractory ALL.
