摘 要:目的探讨2型糖尿病(T2DM)患者血清25-羟维生素D3[25(OH)D3]水平与非酒精性脂肪肝(NAFLD)的相关性.方法选取2017年6月至2018年6月在郑州大学第二附属医院内分泌科就诊的T2DM患者238例,分为单纯T2DM组(111例)和T2DM合并NAFLD组(127例),另选取同期健康体检者30例作为健康对照组.单因素方差分析比较多组间差异,Pearson相关分析法分析血清25(OH)D3与各临床指标的相关关系,Logistic回归分析NAFLD的影响因素.结果T2DM合并NAFLD组25(OH)D3水平低于单纯T2DM组和健康对照组,且单纯T2DM组25(OH)D3水平低于健康对照组,差异有统计学意义(P<0.05).25(OH)D3与非酒精性脂肪肝(r=-0.154,P=0.018)、HOMA-IR(r=-0.161,P=0.013)、TG(r=-0.148,P=0.022)、糖尿病病程(r=-0.161,P=0.013)呈负相关.25(OH)D3是NAFLD的保护因素(P=0.006,95%CI:0.848~0.967).结论2型糖尿病患者中,血清25(OH)D3水平与NAFLD相关,并且是其保护因素.Objective To investigate the correlation between serum 25-hydroxyvitamin D3,that is 25(OH)D3,level and non-alcoholic fatty liver disease(NAFLD)in patients with type 2 diabetes mellitus(T2DM).Methods A total of 238 patients with T2DM were selected from the Department of Endocrinology in the Second Affiliated Hospital of Zhengzhou University from June 2017 to June 2018.They were divided into simple T2DM group(n=111)and T2DM with NAFLD group(n=127).In addition,30 healthy persons in the same period were selected as healthy control group.The single-factor variance analysis was used to compare the differences among groups.Pearson conelation analysis was used to analyze the correlation between serum 25(OH)D3 and clinical indicators.Logistic regression analysis was used to analyze the influencing factors of NAFLD.Results The level of 25(OH)D3 in T2DM with NAFLD group was lower than that in simple T2DM group and healthy control group,and the level of 25(OH)D3 in simple T2DM group was lower than that in healthy control group(P<0.05).25(OH)D3 was negatively correlated with nonalcoholic fatty liver disease(r=-0.154,P=0.018),Homeostasis model assessment-insulin resistance(r=-0.161,P=0.013),triglyceride(r=-0.148,P=0.022)and the course of diabetes mellitus(r=-0.161,P=0.013).25(OH)D3 was the protective factor of NAFLD(P=0.006,95%CI:0.848-0.967).Conclusions In patients with T2DM,serum 25(OH)D3 level is related to NAFLD,and it is a protective factor of the disease.
