经直肠入路瘘管部分切除治疗女童感染性直肠会阴瘘的效果观察

摘　　要：目的观察经直肠入路瘘管部分切除治疗女童感染性直肠会阴瘘的临床疗效.方法回顾性分析2014年4月至2018年5月山西省儿童医院收治的26例感染性直肠会阴瘘女童的临床资料.年龄7个月~8岁,平均2岁2个月.术前充分肠道准备,所有患儿均行经直肠入路瘘管部分切除术,术后早期肠内营养支持.结果术后随访3个月~4年,26例患儿中,瘘口一期愈合24例(92.3%),感染2例(7.7%),经保守治疗后,于术后3~4周愈合.所有患儿会阴外观均良好,无一例出现大便失禁或污粪.结论经直肠入路瘘管部分切除治疗女童感染性直肠会阴瘘具有损伤小、操作简便、会阴外观满意、复发率低等优势,手术疗效确切,值得进一步推广.Objective 10 observe the clinical effects of transrectal partial fistula resection in the treatment of infective rectoperineal fistula in girls.Methods The clinical data of 26 girls with infective rectoperineal fistula admitted to Shanxi Children's Hospital from April 2014 to May 2018 were retrospectively analyzed.Patients'age ranged from 7 months to 8 years with a mean age of 14 months.Preoperative intestinal preparation was adequate.All girls undenvent partial fistula resection via transrectal approach and early enteral nutrition support after operation.Results Following up for 3 months to 4 years after operation,24 of the 26 cases(92.3%)had primary healing of fistula and 2 cases(7.7%)had infection.After conservative treatment,the fistula healed 3 to 4 weeks after operation.All the children had good appearance of perineum,and none of them had fecal incontinence or dirty feces.Conclusions Transrectal partial fistula resection for infectious rectoperineal fistula in girls has the advantages of less injury,simple operation,satisfactory appearance of perineum and low recurrence rate.The operative effect is definite and worth further promotion.
