摘 要:目的观察T1DM大鼠胃组织中缺氧诱导因子1α(HIF⁃1α)和VEGF表达变化及其对胃排空的影响。方法32只雄性SD大鼠用STZ复制T1DM动物模型,随机分为T1DM4周组(DM4W)、T1DM 12周组(DM12W),每组各16只。另16只大鼠为对照组(Con),以100 mmol/L柠檬酸盐缓冲液(pH 4.5)等量腹腔注射。分别于4周、12周后处死DM4W及DM12W组大鼠,测定体重、血糖、胃酸分泌及胃内色素残留率。光镜下观察胃形态学变化。免疫组织化学染色、RT⁃PCR和Western blot法检测HIF⁃1α和VEGF的表达。结果DM4W及DM12W组胃内色素残留率较Con组增加[(48.34±2.44)%vs(58.3±4.15)%vs(39.46±1.79)%,P<0.05],胃壁细胞明显减少,空泡变性。与Con组比较,DM4W及DM12W组胃酸分泌减少(P<0.05),缺氧诱导因子1α和VEGF表达随病程延长增加。结论HIF⁃1α和VEGF在胃黏膜中高表达,在糖尿病胃轻瘫的发生发展中起重要作用。Objective To observe the changes of hypoxia inducible factor⁃1α(HIF⁃1α)and vascular endothelial growth factor(VEGF)in gastric tissue of diabetic rats and their effects on gastric emptying.Methods 32 Sprague⁃Dawley diabetic rats model were induced by a single intraperitoneal injection of 65 mg/kg streptozotocin and were randomly divided into T1DM 4 weeks group(DM4W)and T1DM 12 weeks group(DM12W),with 16 rats in each group.Rats in DM4W and DM12W group were sacrificed at 4 and 12 weeks,respectively.Another 16 rats were set as a control group(Con)and injected intraperitoneally with 100 mmol/L citrate buffer(pH 4.5).The body weight,blood glucose,gastric acid secretion and gas⁃tric emptying rate of the rats were measured.Morphologic changes in gastric mucosa was observed by light microscope.Expression of HIF⁃1αand VEGF were assessed using immunohistochemistry,RT⁃PCR and Western blot analyses.Results Compared with Con group,the rate of gastric residual pigment was signif⁃icantly increased in diabetic rats[(48.34±2.44)%vs(58.3±4.15)%vs(39.46±1.79)%,P<0.05].In T1DM rats,the number of parietal cells and the secretion of gastric acid were obviously decreased and vacuolar degeneration occurred(P<0.05).The expressions of HIF⁃1αand VEGF were significantly in⁃creased with prolonged disease duration in diabetic group.Conclusion The over expressions of HIF⁃1αand VEGF in the gastric mucosa may play a pivotal role in the progression of diabetic gastroparesis.
