房颤患者射频消融术后基质金属蛋白酶-9及左房结构变化的研究

摘　　要：目的探讨房颤患者射频消融术后基质金属蛋白酶-9(MMP-9)及左房结构的变化。方法选取行导管射频消融手术的房颤患者82例,根据随访结果分为手术成功组69例(84.1%)、复发组13例(15.9%),分别于术前及术后1、3个月检测2组的MMP-9,测定左房前后径、左房最大容积、左房最小容积,计算左房容积指数,比较上述指标变化特点。结果射频消融术前,复发组的MMP-9、 LAVmax、LAVmin、 LAVI高于手术成功组,差异有统计学意义(P<0.05)。射频消融术后,成功组患者在术后1、3个月上述指标下降,差异有统计学意义(P<0.05);复发组在术后1个月时MMP-9较术前下降差异有统计学意义(P<0.05),LAVmax、LAVmin、LAVI较术前有所减低,但差异无统计学意义,且术后3个月MMP-9指标上升,较术后1个月增高(P<0.05),LAVmax、 LAVmin、LAVI无明显变化。结论 MMP-9水平及左房容积指标对于房颤患者预测和评估射频消融手术的成功有重要临床价值。Objective To investigate the changes of matrix metalloproteinase-9(MMP-9) level and the left atrial structural in patients with atrial fibrillation after radiofrequency ablation.Methods 82 cases of patients with atrial fibrillation who underwent the catheter radiofrequency ablation were enrolled. According to the follow-up results, the patients were classified into the successful group(n=69,84.1%) and the recurrence group(n=13,15.9%). MMP-9,Left atrial maximum volume(LAVmax),left atrial minimum volume(LAVmin) and left atrial volumeindex(LAVI) were measured respectively before operation,1 month after operation and 3 months after the operation. The volume index of left atrim was calculated.Results The MMP-9,LAVmax,LAVmin and LAVI in the recurrence group were higher than that in successful group before ablation(P<0.05). The above indexes were significantly decreased in successful group at 1 month and 3 months after the ablation(P<0.05). MMP-9 in the recurrence group was decreased at 1 month after the ablation(P<0.05). However,MMP-9 increased at 3 months after the ablation,which was higher than that at 1 months after the ablation(P<0.05). LAVmax,LAVmin and LAVI decreased slightly than those before ablation,but the difference was not significant(P>0.05). The LAVmax,LAVmin and LAVI were not significantly changes at 3 months after ablation.Conclusions MMP-9 and the left atrial volume index are important to predicte and assess the success of radio-frequency catheter ablation for patients with atrial fibrillation.
