摘 要:目的探讨肾脏叶间动脉血流参数与慢性肾脏病(CKD)患者肾功能的相关性。方法选择收治的150例慢性肾脏病患者,采用多普勒超声检测仪,分别测量患者肾脏叶间动脉收缩期峰值流速(Vmax),舒张期末流速(Vmin)及阻力指数(RI),并分析肾脏叶间动脉血流速度及RI与患者肾小球滤过率(eGFR)的相关性。结果 CKD 2期、CKD 3期、CKD 4期患者的Vmax、Vmin及eGFR与对照组比较差异均有统计学意义(P<0.05),CKD 3期、CKD 4期患者的RI与对照组比较差异有统计学意义(P<0.05),且CKD 2期、CKD 3期、CKD 4期患者的各项指标间比较差异均有统计学意义(P<0.05);Vmax、Vmin与eGFR呈正相关性(r=0.47,0.62);RI与eGFR呈负相关性(r=-0.38);RI升高组与RI正常组患者的年龄、Vmin、RI、Ccr、eGFR、Scr差异均有统计学意义(P<0.05)。结论 CKD患者叶间动脉血流速度与eGFR呈正相关性,RI与eGFR呈负相关性,因此采用多普勒超声检查测定叶间动脉血流参数可作为诊断CKD患者肾功能状态的有效方法。Objective To investigate the correlation between renal interlobar artery blood flow parameters and renal function in patients with chronic kidney disease.Methods 150 patients with chronic kidney disease were selected.The peak systolic velocity(Vmax),end diastolic velocity(Vmin) and resistance index(RI) of the renal interlobar arteries were measured by Doppler ultrasonography,and the correlation between blood flow velocity of the interlobar arteries,RI and glomerular filtration rate(eGFR) was analyzed.Result Vmax,Vmin and eGFR of patients with CKD stage 2,CKD stage 3 and CKD stage 4 were significantly different from those of the control group(P<0.05). RI of patients with CKD stage 3 and CKD stage 4 were significantly different from those of the control group(P<0.05),and the CKD2,CKD3,and CKD stage 4 patients were statistically significant(P<0.05). Vmax,Vmin were positively correlated with eCFR(r=0.47,0.62). RIwere negative correlation with eCFR(r=-0.38). Age,Vmin,RI,Ccr,eGFR,Scr were significantly different between RI elevation group and RI normal group(P<0.05).Conclusion The interstitial blood flow velocity of CKD patients is positively correlated with eGFR,and RI is negatively correlated with eGFR. Therefore,using Doppler ultrasonography to determine inter-arterial blood flow parameters is an effective method for diagnosing renal function in patients with chronic kidney disease.
