男性高频多次机采血小板献血者补充善存片铁储备状况的研究

摘　　要：目的研究男性高频多次机采血小板献血者(连续3年,每年献机采≥18次,且总献血次数≥60次)补充善存佳维片1年前后的血清铁蛋白(SF)、血红蛋白(Hb),血清铁(SI),总铁结合力,转铁蛋白饱和度的变化情况。方法随机抽取本中心男性高频多次机采血小板献血者60例,随机分为2组,未服善存片组和服用善存片组(每日补充善存片1片,补充1年),献血者继续高频多次机采献血,留取2017年第1次和最后1次机采前的静脉血,检测2组献血者Hb和铁代谢指标含量并统计分析。结果2组献血者2017年第1次和最后1次机采时,SF和Hb,SI,总铁结合力,转铁蛋白饱和度的平均值都在正常范围;2组献血者2017年最后1次机采时,未服善存片组与服用善存片组SF平均值(μg/L)分别为37.5±17.5和46.1±15.2,2组比较采用独立样本T检验,t=2.01,P<0.05;2017年末次献血,未服善存片组SF<12μg/L的比率为20%(6/30),服用善存片组SF<12μg/L的比率为3.3%(1/30),x2=4.04,P<0.05。结论男性高频多次机采血小板献血者SF水平会随着献血总次数和频率的增加而逐渐减少;每日服用善存佳维片1片,能有效稳定高频多次机采献血者SF水平,这对于献血者长期高频多次机采献血有着重要的意义,采血机构应该更加重视对高频多次机采血小板献血者SF的检测和铁剂补充的宣传和研究。Objective To assess the changes of serum ferritin(SF),hemoglobin(Hb),serum iron(SI),total iron binding capacity,and transferrin saturation levels in male regular plateletpheresis donors after they’ve taken Centrum tablets for one year.Methods A total of 60 male regular plateletpheresis donors,who donated more than 18 times per year,and more than 60 times of total donation during consecutive three years,were randomly selected and divided into two groups to measure to the effect of taking Centrum tablets.Donors from two groups,that is taking Centrum tablets group(one Centrum tablet daily for one year)and non-taking Centrum tablets group,kept plateletpheresis donation at a high frequency.The contents of Hb and iron metabolism index of the first and the last platelet donation in 2017 were detected and analyzed statistically.Results The average values of SF,Hb,SI,total iron binding capacity,and transferrin saturation of the first and last platelet donation in 2017 remained in the normal range in both groups.In the last time of platelet collection in 2017,the average value of SF(μg/L)in non-taking and taking groups was 37.5±17.5 and 46.1±15.2,respectively,which showed significant difference via independent sample T test(t=2.01,P<0.05).Moreover,the ratio of SF less than 12μg/L was 20%(6/30)in the non-taking group and 3.3%(1/30)in the taking group,which showed significant difference via Chi-square test(x2=4.04,P<0.05).Conclusion The SF level of male regular plateletpheresis donors donating at high frequency decreases as donation intervals and total donation times increase.Taking one Centrum tablet daily can effectively stabilize the SF level of them.For such regular plateletpheresis donors,blood service centers should pay more attention to SF detection and strengthen the publicity and research of iron supplements.
