摘 要:目的对不同基础疾病患者的抗体变化情况进行调查研究,发现抗体性质与疾病之间的关联,为临床血型抗体的检测,以及输血提供重要的参考数据。方法对2009—2018年本实验室检出不规则抗体的孕妇和其他有详细病征的患者进行回顾性调查,结合血清学试验结果,进行统计分析。结果非孕妇患者体内产生的不规则抗体中温自身抗体占23%。患者体内同种抗体中,最常见的为Rh抗体,占58%,其次为MNS抗体、Lewis抗体。不同疾病产生抗体的几率不相同,同种疾病男性和女性产生抗体的几率也有差异。自身免疫性溶血性贫血、血液病等较易产生Ig G同种抗体,也较易产生自身抗体,干扰同种抗体的检测。结论不同抗体的特点和患者疾病存在关联,发生率有显著差异,可以为临床血型抗体的检测以及输血提供重要的参考数据,以减少同种抗体的形成和避免输血反应。Objective To investigate the changes of antibodies in patients with different underlying diseases,and to find out the relationship between antibody and diseases,and provide important reference data for the detection of irregular erythrocyte antibodies and blood transfusion.Methods A retrospective investigation of serological test results of our laboratory from2009 to 2018 was conducted to statistically analyze the differences of irregular antibodies between pregnant women and other patients.Results Among the irregular antibodies produced by non-pregnant patients,the warm autoantibodies accounted for23%.The most common antibody in the patients was Rh antibody,accounting for 58%,followed by MNS antibody and Lewis antibody.Different diseases had different rates of antibody production,even the same disease had different rates of antibody production between males and females.Diseases such as autoimmune hemolytic anemia and blood diseases,etc.were more likely to produce Ig G alloantibodies,and also more likely to produce autoantibodies,which were associated with interfere with the detection of alloantibodies.Conclusion The properties of antibodies are related to the type of diseases in patients.The incidence of antibodies is significantly different,which can inspire the detection of irregular antibodies and reducing the adverse reaction of blood transfusion caused by alloantibodies.
