摘  要:目的针对本地区无偿献血者血小板含量普遍偏低、浓缩血小板含量质量抽检不达标,优化手工血小板制备技术,主要提高血小板回收率,为临床提供高质量浓缩血小板。方法对来源于固定献血屋采集的400 m L全血120袋,随机分成2组,每组60袋,对照组采用原参数(离心时间、离心力)制备血小板;优化组采用自调参数(离心时间、离心力)制备血小板。对照组使用的参数来自上海血液中心和离心机使用说明书,优化组在对照组的基础上延长离心时间同时改变离心力,尤其是在手工分离血小板时改变分离管的半径,达到改变血小板分离流速。结果优化组血小板回收率为88%、对照组血小板回收率57%(t=2.46,P<0.05),同时优化组红细胞混入量低于对照组17%(t=2.26,P<0.05)。结论通过优化富浆法制备血小板的离心时间、离心力以及改变分离管的半径等方法,能做到显著提高血小板的回收率,同时能减少红细胞混入量,这种技术可以在其他血站推广使用。Objective To improve the recovery rate of manualplatelet concentratesderived from platelet-rich plasma(PRP)by optimizing thepreparationprotocolaiming atprovidinghighquality platelet concentrates forthe clinical.Methods A total of 120 bags of 400 m L whole blood collected atthe blood collectionsitewere randomly divided into two groups(60 bags pergroup).Platelets in the control group were prepared manually by original protocol(such ascentrifugal time and centrifugal force),whileplatelets in the optimization group were prepared by self-adjustedprotocol,which wereoptimized according to the centrifugemanual and suggestionsgiven by Shanghai Blood Center.Optimalflow rate forseparating PRPfrom whole bloodwas achievedwithprolongedcentrifugal time and adjusted centrifugal force,particularin shorteningthe radius of separation tubeto reduce siphonduring manualpreparation.Results The recovery rate of platelets in the optimization group and control group was88%and 57%(t=2.46,P<0.05),and lesserythrocytes(by17%)were contained inthe optimization group thanin control group(t=2.26,P<0.05).Conclusion The manual platelet concentrates derived from PRPwith optimizedprotocolhada superiorrecovery rate of plateletsand less RBCcontamination,whichdeservesfurtherpromotion.
