摘  要:目的制作急性肺损伤(ALI)模型并观察结合黏附分子-1(JAM-1)和P-选择素(P-selectin)的变化特点,并探讨谷氨酰胺(glutamine,Gin)对其影响。方法SD大鼠随机分对照组、ALI组和谷氨酰胺治疗组。测定肺组织湿干重比值(W/D),酶联免疫吸附法(ELISA)检测肺组织JAM-1;支气管肺泡灌洗液(BALF)中性粒细胞计数、蛋白测定和EUSA法检测JAM-1;免疫组化检测肺组织中性粒细胞髓过氧化物酶(MPO)和肺毛细血管内皮细胞P-selectin;光学显微镜下肺组织形态学改变和电镜下超微结构的变化。结果ALI组和治疗组肺组织JAM-1含量、BALF中蛋白含量、中性粒细胞计数和JAM-1含量均高于对照组(P<0.05),但治疗组低于ALI组(P<0.05)。ALI组大鼠肺组织W/D高于对照组和治疗组(P<0.05)。肺组织中性粒细胞和肺毛细血管内皮细胞P-选择素的表达在ALI组表达为强表达,对照组低水平表达,治疗组介于二者之间。光学显微镜下观察肺组织形态学发现ALI组肺间质明显水肿,红细胞及中性粒细胞明显浸润,对照组肺间质水肿、红细胞及中性粒细胞浸润较轻,治疗组介于二者之间。上述现象在电镜下也得到证实。结论ALI时肺组织JAM-1和P-选择素表达均增加,二者通过增加肺毛细血管膜渗透性和肺组织中性粒细胞数量参与了ALI的发生。Gln对肺损伤有保护作用。Objective To observe the changes and significance of JAM-1 and P-selectin and to explore the role of glutamine in ALI rats.Methods SD rats were randomly divided into 3 groups:the control group,the ALI group and the treatment group,the lung wet/dry ratio(W/D)was calculated.The concentration of JAM-1 was detected by ELISA and the expressions of both P-selectin and PMN was determined by immunohistochemical method.Changes of lung histomorphology and ultrastructure under optical microscope and electron microscope.Results The concentrations of both protein and JAM-1 in lung tissues and W/D in lung tissues in treatment group were higher(P<0.05)than those in control group,and were lower than those in ALI group(P<0.05).P-selectin and MPO expressions in lung tissues of ALI group were distributed mainly in pulmonary capillary endothelial cells.From low to high levels,the order was control group<treatment group<ALI group.Conclusion In ALI induced by LPS,JAM-1 and P-selectin are significantly increased and involved in the occurrence and development of ALI.Glutamine plays an important role in the prevention and treatment of acute lung injury.
