两种微创手术治疗腰椎间盘突出症疗效及VAS评分和ODI指数改变的对比研究

摘　　要：目的分析腰椎间盘突出症采用不同微创手术方式治疗的临床效果。方法于南阳市第一人民医院2017年2月至2019年4月期间收治的腰椎间盘突出症患者中选取70例,根据手术方式不同随机分为两组,A组采用经皮椎间孔镜手术,B组采用微创通道手术治疗,比较两组治疗效果。结果A组患者的切口长度为(0.7±0.2)cm,术中出血量为(18.2±3.1)mL,住院时间为(6.2±1.3)d,分别与对照组的(2.5±0.6)cm、(46.4±5.7)mL和(10.8±1.9)d比较,差异有统计学意义(P<0.05)。两组手术时间比较差异不明显,对比无统计学意义(P>0.05)。A组术后腰腿痛VAS评分为(1.8±0.3)分,显著低于B组的(2.4±0.6)分,差异明显。A组与B组术后0DI指数比较,无显著性差异(P>0.05)。A组与B组治疗优良率分别为94.2%和91.4%,差异无统计学意义(P>0.05)。结论经皮椎间孔镜手术和微创通道手术在腰椎间盘突出症治疗中均具有较好疗效,但经皮椎间孔镜手术给患者造成的创伤更小,患者术后恢复更快,具有更高安全性。Objective To analyze the clinical effects of different minimally invasive surgical methods for lumbar disc herniation.Methods Totally 70 patients with lumbar disc herniation admitted to our hospital from February 2017 to April 2019 were selected and divided into two groups.Group A received percutaneous intervertebral foraminal surgery and group B received minimally invasive channel surgery to compare the therapeutic effects of the two groups.Results The incision length was(0.7±0.2)cm,the intraoperative blood loss was(18.2±3.1)mL,and the hospital stay was(6.2±1.3)days,which were significantly different from the(2.5±0.6)cm,(46.4±5.7)mL and(10.8±1.9)days in the control group(P<0.05).There was no significant difference in operation time between the two groups(P>0.05).VAS score of postoperative lumbar and leg pain in group A was(1.8±0.3),which was significantly lower than(2.4±0.6)in group B.There was no significant difference in the postoperative ODI index between group A and group B(P>0.05).The excellent and good rates of treatment in group A and group B were 94.2% and 91.4%,respectively,with no statistically significant difference(P>0.05).Conclusion Percutaneous intervertebral foramen surgery and minimally invasive channel surgery have good efficacy in the treatment of lumbar disc herniation,but percutaneous intervertebral foramen surgery causes less trauma to patients,patients recover faster after surgery,and has higher safety.
