摘 要:目的探究磁共振全身弥散加权成像在肿瘤诊断方面的临床应用价值。方法选择我院自2016年5月至2017年5月收治的87例肿瘤患者作为研究对象,所有患者均行磁共振全身弥散加权成像诊断(WB-DWI)、全身骨显像扫描诊断(ECT)以及手术病理检查,比较WB-DWI与ECT检测出的原发灶、转移灶和总阳性灶个数、对恶性肿瘤全身转移患者诊断的敏感性、准确性,对比WB-DWI检测出的良恶性肿瘤ADC值。结果经诊断,ECT诊断有83例患者与手术病理检查结果一致,诊断准确率为95.4%,WB-DWI诊断有84例患者与手术病理检查结果一致,诊断准确率为96.6%,两组对比无显著差异(P>0.05);WB-DWI诊断对肿瘤原发灶、转移灶和总阳性灶的检出率明显高于ECT诊断,两组对比差异显著(P>0.05);WB-DWI与ECT对恶性肿瘤全身转移患者诊断的敏感性、准确性对比无显著差异(P>0.05);WB-DWI检测出恶性肿瘤患者的平均ADC值(1.1±0.5)明显低于良性肿瘤患者的平均ADC值为(2.5±0.6),两组数据对比差异显著(P>0.05)。结论WB-DWI与ECT在肿瘤疾病诊断中均具有良好的应用价值,均可作为疾病诊断的重要依据,但WB-DWI诊断对原发灶、转移灶和总阳性灶肿瘤的检出率更高,且可通过WB-DWI检查测得的肿瘤ADC值,鉴别和区分患者肿瘤的良、恶性。Objective To investigate the clinical value of whole body diffusion-weighted MR imaging in the diagnosis of tumors.Methods Totally 87 patients with cancer who were treated in our hospital from May 2016 to May 2017 were selected as the study subjects.All patients underwent MRD(WB-DWI),whole body bone scan imaging(ECT),and pathological examination.The sensitivity,accuracy of WB-DWI and ECT in the diagnosis of patients with systemic metastasis of malignant tumors were compared with those detected by WB-DWI and ECT.Malignant tumor ADC value.Results After diagnosis,83 patients with ECT were consistent with the pathological findings.The diagnostic accuracy was 95.4%.The results of WB-DWI in 84 patients were consistent with the results of surgery and pathology.The diagnostic accuracy was 96.6%(P>0.05).The detection rate of WB-DWI in primary tumors,metastases and total positive lesions was significantly higher than that in ECT(P>0.05).The sensitivity and accuracy of ECT in the diagnosis of patients with systemic metastasis of malignant tumor had no significant difference(P>0.05).The average ADC value of malignant tumor detected by WB-DWI(1.1±0.5)was significantly lower than that of benign tumor(2.5±0.6),the difference between the two groups was significant(P>0.05).Conclusion Both WB-DWI and ECT have good value in the diagnosis of tumor,both of which can be used as an important basis for disease diagnosis.However,the detection rate of WB-DWI in primary tumor,metastatic tumor and total positive tumor is more High,and can be measured by WB-DWI tumor ADC value,identify and distinguish between patients with benign and malignant tumors.
