ADC值直方图分析方法鉴别IB期和Ⅱ期宫颈癌的应用初探

摘　　要：目的探讨MR DWI ADC直方图分析法鉴别IB期和II期宫颈癌的应用价值。方法回顾性分析经手术病理证实但未接受过治疗的42例宫颈癌患者,依照国际妇产科联盟分期方法进行分期并以此进行分组,IB期和II期各21例。两组患者术前均行常规盆腔MRI平扫、DWI及动态增强扫描,采用后处理软件生成ADC值,选取两组肿块ADC图像最大层面,运用Mazda软件勾画感兴趣区并行直方图分析。对两组数据直方图参数特征进行统计学分析,找出两组之间有显著性差异的参数特征。结果两组ADC值直方图以偏态分布为主,利用直方图提取出9个参数(平均值、方差、偏度、峰度、ADC1%、ADC10%、ADCmean、ADC90%、ADC99%),其中峰度值最有统计学意义(P<0.05)。受试者工作特征曲线下面积为0.753,最佳截断点值为51.5,诊断的敏感度和特异度分别为80.1%和71.4%。结论ADC值直方图评估宫颈癌IB期和II期具有较大价值。Objective To investigate the diagnostic value of ADC histogram obtained from MR DWI for stage IB and II cervical cancer.Methods A retrospective analysis was made of 42 cases of cervical cancer confirmed by operation and pathology but not treated.According to the International Federation of Gynecology and Obstetrics staging method,these cases were divided into two groups,21 cases in stage IB and 21 cases in stage II.Routine pelvic MR sequences,DWI and dynamic contrast-enhanced scan were performed in both groups before operation.ADC values were generated by post-processing software,and the maximum levels of ADC images of two groups of masses were selected.The region of interest was delineated by Mazda software and analyzed by histogram.Statistical analysis of the histogram parameters of the two groups was carried out to find out the parameters with significant differences between the two groups.Results The histograms of ADC values in the two groups were mainly skewed distribution.Nine parameters(mean,variance,skewness,kurtosis,ADC1%,ADC10%,ADCmean,ADC90%,ADC99%)were extracted by histogram,of which the kurtosis was the most significant(P<0.05).The area under the receiver operating characteristic curve was 0.753,and the best cut-off point was 51.5.The sensitivity and specificity of diagnosis were 80.1%and 71.4%respectively.Conclusion ADC histogram is of great value in evaluating stage IB and stage II of cervical cancer.
