基于扶阳抑阴理论运用吴氏长蛇灸治疗CFS患者轻度抑郁状态的临床研究

摘　　要：目的:客观评价吴氏长蛇灸对脾肾两虚型慢性疲劳综合症(CFS)患者抑郁状态的改善情况,为扶阳抑阴理论在抑郁症中的治疗提供临床证据。方法:采用中央随机,将符合纳入标准的CFS患者随机分为吴氏长蛇灸结合针刺组(n=51)和单纯针刺组(n=61)。治疗频率为3次/周,连续治疗8周。治疗前及结束时,将抑郁自评量表(self-rating depression scale,SDS)评分53分以上的CFS患者进行SDS量表积分、疗效自评及脾肾两虚中医证候积分评价。结果:吴氏长蛇灸结合针刺能有效改善CFS患者疲乏、抑郁(P=0.003)及中医证候症状(P=0.001),且相对于单纯使用针刺,吴氏长蛇灸结合针刺在改善轻度抑郁方面有优势(P=0.021)。结论:在相同治疗期待值下,吴氏长蛇灸能有效改善CFS伴轻度抑郁状态患者的疲劳及抑郁状态,说明扶阳抑阴思想应贯穿在抑郁症治疗始终。Objective:To investigate the therapeutic efficacy of acupuncture for Wu’s Du vessel moxibustion for treating chronic fatigue syndrome with mild depression,to provide evidence of Yang-supporting-and-Yin-abating theory in the treatment for depression.Methods:Central-randomanization controlled trial was designed.Patients meeting the inclusion criteria were randomly allocated to treatment group(Wu’s Du vessel moxibustion and acupuncture,n=51)or control group(acupuncture,n=61).The study consisted of a treatment period of 8 weeks(three sessions per week).Before and at the end of treatment,the CFS patients with self-rating Depression Scale(SDS)score of more than 53 were assessed with SDS,Self-report overall response and the scale of Spleen and Kidney Qi Deficiency.Results:Wu’s Du vessel moxibustion combined with acupuncture could effectively improve fatigue,depression symptoms(P=0.003)and TCM syndrome symptoms(P=0.001)of CFS patients.Compared with acupuncture alone,Wu’s Du vessel moxibustion combined with acupuncture had advantages in improving mild depression(P=0.021).Conclusion:Wu’s Du vessel moxibustion is effective in the treatment of depression,which indicates the importance of Yang-supporting and Yin-abating in the treatment for depression.
