摘　　要：目的分析布鲁菌病合并EB病毒(EBV)感染患者的临床特点,实验室检查及诊治情况。方法回顾性分析2017年1月至2018年12月哈尔滨医科大学附属第一医院感染科收治的114例急性布鲁菌病住院患者的临床资料,分析并总结EBV感染阳性及阴性患者的临床特点。结果在入组的114例急性布鲁菌病感染患者中,EBV阳性41例,占35.96%。EBV阳性组患者的关节痛和肌肉痛比例明显低于EBV阴性组[34.1%(14/41)比64.4%(47/73),46.3%(19/41)比67.1%(49/73)](P<0.05)。EBV阳性组患者的白细胞计数明显低于EBV阴性组[(4.6±1.9)×109/L比(5.4±1.9)×109/L](P<0.05)。二分类反应变量的Logistic回归分析结果显示,年龄越大(调整后OR=1.285,95%CI 1.016~1.624,P=0.036)和肌酸激酶升高(调整后OR=1.158,95%CI 1.004~1.335,P=0.044)是住院期并发症发生的影响因素。结论布鲁菌病患者可以导致EBV激活,但并没有因为合并EBV感染而导致病情加重。Objective To investigate the clinical characteristics,laboratory examination,diagnosis and treatment of brucellosis complicated with Epstein-Barr virus(EBV)infection.Methods The data of 114 inpatients with acute brucellosis with positive and negative EBV infection in the Department of Infectious Diseases,the First Affiliated Hospital of Harbin Medical University from Jan.2017 to Dec.2018,were retrospectively analyzed to summarize the clinical characteristics of the EBV positive and negative patients.Results Among the 114 patients with acute brucellosis,41 were EBV positive,accounting for 35.96%of the total.The rates of arthralgia and muscle soreness in the EBV positive group were significantly lower than those in the EBV negative group[34.1%(14/41)vs 64.4%(47/73),46.3%(19/41)vs 67.1%(49/73)](P<0.05).The leukocyte count in the EBV positive group was significantly lower than that in the EBV negative group[(4.6±1.9)×109/L vs(5.4±1.9)×109/L](P<0.05).The results of binary Logistic regression analysis showed that the older the age(adjusted OR=1.285,95%CI 1.016-1.624,P=0.036)and the higher the cretin kinas(adjusted OR=1.158,95%CI 1.004-1.335,P=0.044)were influnce of the complications during hospitalization.Conclusion Brucellosis can cause EBV activation,but it does not aggravate because of EBV infection.
