摘　　要：目的探索紧密型医联体模式对二级甲等综合性公立医院发展的影响。方法在政府与政策的支持下与新疆医科大学第一附属医院建立紧密型医联体,收集该模式下2016—2018年昌吉分院的医疗运行与服务、医院感染与传染病管理数据,分析平均发展速度,观察发展趋势。结果2016—2018年昌吉分院入院与出院患者人次、手术台次、床位使用率、危重收治率、临床路径覆盖率与完成率、三级医师查房合格率、不良事件上报例数、传染病网报例数呈逐渐上升趋势,平均住院日、术前占床日与医院感染现患率呈逐年缩短趋势;临床路径覆盖率与不良事件上报例数显著增加,平均发展速度分别为142.4%、146.3%。结论实施紧密型医联体管理模式,可有效实现优质医疗资源下沉,提升基层医院医疗服务能力和水平,为群众提供更加优质、高效、便捷的医疗卫生服务。Objective:To explore the influence of compact medical consortium model on the development of comprehensive public hospitals of class one and grade two.Method:The First Affiliated Hospital of Xinjiang Medical University established a compact medical consortium with the support of the government and policies.Data of medical operation and service,hospital infection and infectious disease management in Changji Branch from 2016 to 2018 were collected under this model,the average development speed was analyzed and the development trend was observed.Result:From 2016 to 2018,the number of patients admitted and discharged from Changji branch hospital,the number of operation tables,bed utilization rate,critical care admission rate,clinical pathway coverage rate and completion rate,the qualified rate of three-level physicians’house rounds,the number of the reported adverse events and the number of online reported infectious diseases showed a gradual rising trend.The average length of hospital stay,the number of days in bed before surgery and the incidence of nosocomial infection decreased year by year.Clinical pathway coverage and the reported adverse events increased significantly,with an average development rate of 142.4% and 146.3%,respectively.Conclusion:The implementation of compact medical consortium management mode can effectively realize the sinking of high-quality medical resources,improve medical service capacity and level of grassroots hospitals,and provide more high-quality,efficient and convenient medical and health services for the masses.
