持续正压通气在急性左心衰治疗中作用观察

摘　　要：目的探讨持续正压通气在急性左心衰治疗中的应用价值。方法选取本院2017年2月至2019年2月收治的60例急性左心衰患者,按随机数字表达法分为研究组(30例,常规内科治疗+持续正压通气)与对照组(30例,常规内科治疗+机械通气),观察两组治疗前、后动脉血气分析指标、心率(HR)等指标变化。结果与治疗前比较,两组血氧分压(PaO2)、血氧饱和度(SaO2)水平显著提高、HR与二氧化碳分压(Pa-CO2)水平显著下降(P<0.05),两组比较差异无统计学意义;两组治疗总有效率比较差异无统计学意义;研究组未见并发症,对照组出现6例并发症,两组比较差异有统计学意义(P<0.05)。结论持续正压通气辅助内科常规治疗能够快速纠正急性左心衰患者缺氧状态,改善心功能,且安全性更高。Objective To explore the value of continuous positive airway pressure in the treatment of acute left heart failure.Methods Sixty patients with acute left heart failure admitted to our hospital from February 2017 to February 2019 were randomly divided into study group(30 cases,routine medical treatment+continuous positive pressure ventilation)and control group(30 cases,routine medical treatment+Mechanical ventilation),observe changes in indicators such as arterial blood gas analysis indicators and heart rate(HR)before and after treatment.Results Compared with before treatment,the levels of partial oxygen pressure(PaO2)and blood oxygen saturation(SaO2)were significantly increased,and the levels of HR and partial carbon dioxide pressure(PaCO2)were significantly decreased(P<0.05).There was no significant difference in the total effective rate of treatment between the two groups;no complications were found in the study group,and 6 complications occurred in the control group.There was significant difference between the two groups(P<0.05).Conclusion Continuous positive pressure ventilation assisted medical routine treatment can quickly correct the hypoxic state of patients with acute left heart failure,improve cardiac function,and have higher safety.
