经皮微创钢板内固定（MIPPO）技术治疗闭合性胫骨下端骨折的疗效分析

摘　　要：目的探究经皮微创钢板内固定(MIPPO)技术治疗闭合性胫骨下端骨折的疗效。方法选取2017年5月至2019年5月本院收治的80例闭合性胫骨下端骨折患者作为研究对象。使用抽签法进行分组处理,40例纳入观察组,采用经皮微创钢板内固定(MIPPO)技术治疗;40例纳入对照组,采用单一钢板内固定治疗。观察对比两组患者治疗效果、不良反应发生情况、治疗前后疼痛度、生活质量等。结果观察组1例患者疗效不明显,1例病情恶化,总有效率95.0%,对照组8例患者疗效不明显,2例病情恶化,总有效率75.0%,两组比较差异有统计学意义(P<0.05);记录患者不良反应,观察组中1例头晕,1例头痛不良反应,发生率5.0%,对照组中5例头晕,4例头痛,不良反应发生率22.5%,两组比较差异有统计学意义(P<0.05)。两组患者治疗前疼痛度明显,评分均在5分以上,患者自述疼痛度高,治疗完成后,患者均表示疼痛情况得到缓解,评分明显低于治疗前,两组治疗后的评分比较差异有统计学意义(P<0.05);生活质量的观察中,观察组患者的生理职能评分、生理机能评分等,与对照组患者比较差异有统计学意义(P<0.05)。结论对闭合性胫骨下端骨折的患者采用经皮微创钢板内固定(MIPPO)技术治疗,不仅显著提升治疗有效率,缓解患者疼痛,并且此法对患者造成的伤害较小,不良反应发生率低,改善患者生活质量,临床应用效果理想,值得推广。Objective To investigate the effect of minimally invasive percutaneous plate osteosynthesis(MIPPO)in the treatment of closed distal tibial fractures.Methods 80 patients with closed distal tibial fracture in our hospital were investigated by comparative experimental study.The study started in May 2017 to 2019.40 cases were treated with minimally invasive percutaneous plate internal fixation(MIPPO)and 40 cases were treated with single plate internal fixation.The therapeutic effect,adverse reactions,pain degree and quality of life before and after treatment were observed and compared between the two groups.Results The total effective rate was 95.0%in 1 case of observation group and 1 case of deterioration.The total effective rate was 75.0%in 8 cases of control group and 2 cases of deterioration.The difference between the two groups was significant(P<0.05).The adverse reactions of patients were recorded.The incidence of dizziness and headache was 5.0%,1 case of observation group and 5 cases of control group.The incidence of dizziness and headache was 22.5%.There was significant difference between the two groups(P<0.05).Two groups of patients had obvious pain degree before treatment,the score was above 5 points,the patient's self-reported pain degree was high.After treatment,the patients all indicated that the pain situation had been alleviated,the score was significantly lower than that before treatment,and the score after treatment was also significantly different between the two groups(P<0.05).In the observation of quality of life,the patients in the observation group had significant difference(P<0.05).In the observation of quality of life,the physiological function scores and physiological function scores of the observation group were statistically significant(P<0.05).Conclusion Percutaneous minimally invasive plate internal fixation(MIPPO)for closed distal tibial fractures can not only significantly improve the treatment efficiency and relieve pain,but also cause less injury,lower incidence of adverse reactions,i
