摘　　要：目的分析老年高血压患者血压控制的影响因素,为高血压的预防控制提供思路。方法选择2016年12月至2018年12月首都医科大学宣武医院收治的793例高血压患者作为研究对象,所有患者均常规采用两种或两种以上药物联合治疗,治疗8周后依据《2010年中国高血压防治指南》判定标准分为控制达标组和控制不达标组,自制调查问卷对所有患者进行面对面问卷调查。结果793例高血压患者血压控制达标率为30.64%(243例)。两组患者年龄、病程、体重指数、运动、高盐饮食、高脂饮食、合并症情况以及按时服药情况等差异有显著性(P<0.05)。多因素logistic回归分析显示,年龄≥70岁、病程≥10年、体重指数≥24kg/m2、不规律运动、高盐饮食、高脂饮食和未按时服药均是老年高血压控制的影响因素。结论本院老年高血压患者血压控制达标率偏低,影响血压控制的因素是多方面的,因此在治疗中需关注高龄、病程较长、肥胖症患者,同时规范其饮食起居。Objective To analyze the influencing factors of blood pressure control in elderly hypertensive patients and provide ideas for the prevention and control of hypertension.Method A total of 793 hypertensive patients admitted to Xuanwu Hospital of Capital Medical University from December 2016 to December 2018 were selected as subjects.All patients were treated with two or more drugs.After 8 weeks of treatment,according to the 2010 China Guidelines for the Prevention and Treatment of Hypertension,the criteria were divided into control compliance group and control failure standard group.Self-made questionnaires are used to conduct face-to-face questionnaire surveys for all patients.Result The blood pressure control compliance rate of 793 patients with hypertension was 30.64%(243 cases).There were significant differences in age,patient duration,body mass index,exercise,high-salt diet,combination medication,and on-time medication(P<0.05).Multivariate logistic regression analysis showed that age≥70 years,duration≥10 years,Body mass index≥24kg/m2,irregular exercise,high-salt diet,high-fat diet and not taking medication on time are the influencing factors of hypertension control in the elderly.Conclusion The blood pressure control compliance rate of elderly hypertensive patients in this hospital is low,and the factors affecting blood pressure control are various.Therefore,it is necessary to pay attention to the elderly,long-term disease,obesity patients,and regulate their diet.
