摘　　要：目的:探讨住院期间接受血液净化治疗的急性肾损伤(AKI)患者的临床特点及肾脏预后情况。方法:回顾性分析2012年08月~2016年07月收治的AKI患者,按治疗方式分为常规治疗组(n=133)和血液净化组(n=71),按照肾脏预后将血液净化组分为肾功能恢复组(n=28)和肾功能未恢复组(n=43)。记录人口统计学资料、基础疾病、血清学指标等情况,分析AKI患者的临床特点及肾脏预后情况。结果:共有204例AKI患者纳入研究,平均年龄(55.96±20.84)岁。常规治疗组和血液净化组心力衰竭、AKI分期、eGFR、APACHEⅡ评分、平均住院日、出院时肾功能恢复人数比较差异具有统计学意义(P<0.05)。出院时,常规治疗组肾功能恢复100例(75.2%),血液净化组肾功能恢复28例(39.4%)。多因素Logistic回归分析显示,年龄和血红蛋白是血液净化组AKI肾脏预后的预测因素。结论:血液净化组AKI肾脏预后不良与确诊时肾功能差相关。年龄和血红蛋白是血液净化组AKI肾脏预后的预测因素。Objective:To investigate the clinical characteristics and prognosis of patients with acute kidney injury(AKI)treated with blood purification during hospitalization.Methods:The patients with AKI treated from August 2012 to July 2016 were retrospectively analyzed.According to the treatment,the patients were divided into routine treatment group(n=133)and blood purification group(n=71),and patients in the blood purification group were further assigned to renal function recovery group(n=28)and renal function non-recovery group(n=43)according to renal prognosis.Demographic data,underlying diseases,serological indicators,etc.were recorded to analyze the clinical characteristics and renal prognosis of patients with AKI.Results:A total of 204 AKI patients were enrolled in this study,with an average age of(55.96±20.84)years.There were significant differences in heart failure,AKI stage,estimated glomerular filtration rate(eGFR),APACHEⅡscore,average hospitalization days and the number of renal function recovery at discharge between the routine treatment group and the blood purification group(P<0.05).100 cases(75.2%)were recovered in the routine treatment group and 28 cases(39.4%)were recovered in the blood purification group at discharge.Multivariate logistic regression analysis showed that age and hemoglobin were predictorsof AKI renal prognosis in the blood purification group.Conclusion:The poor prognosis of AKI in blood purification group was associated with poor renal function at diagnosis.Age and hemoglobin were predictorsof AKI renal prognosis in the blood purification group.
