摘　　要：目的比较3种溶栓方法治疗急性脑梗死的临床效果。方法回顾性分析我科2016年1月至2018年12月收治的227例急性脑梗死患者的临床资料,按照治疗方法不同将其分为A组(106例,静脉溶栓)、B组(73例,颈内动脉溶栓)和C组(48例,血栓内接触性溶栓)。比较三组的临床效果。结果治疗后2、24 h、7 d,B组的NIHSS评分低于A组,C组的NIHSS评分低于A组和B组(P<0.05)。C组的治疗总有效率和血管总再通率均优于A组和B组,B组均优于A组(P<0.05)。三组的总出血率无显著差异(P>0.05)。结论血栓内接触性溶栓治疗急性脑梗死有起效快、并发症少、血管再通率及安全性高的优势。Objective To compare the clinical effect of three thrombolytic methods in the treatment of acute cerebral infarction. Methods The clinical data of 227 patients with acute cerebral infarction admitted in our department from January 2016 to December 2018 were analyzed retrospectively, and the patients were divided into group A(106 cases, intravenous thrombolysis), group B(73 cases, thrombolysis of the internal carotid artery) and group C(48 cases, thrombolysis by contact within thrombus) according to the different treatment methods. The clinical effects of the three groups were compared. Results The NIHSS score in the group B was lower than that in the group A, that in the group C were lower than the group A and the group B at 2, 24 hours, and 7 days after treatment(P<0.05). The total effective rate of treatment and total recanalization rate in the group C were better than those in the group A and the group B, and those in the group B were better than the group A(P<0.05). There was no significant difference in the total bleeding rate among the three groups(P>0.05). Conclusion Thrombolysis by contact within thrombus in the treatment of acute cerebral infarction has the advantages of quick onset,less complications, high recanalization rate and safety.
