摘　　要：目的探究Twin-block阻鼾器在老年阻塞性睡眠呼吸暂停低通气综合征合并冠心病患者中的应用效果。方法选取2015年1月至2018年12月我院收治的老年阻塞性睡眠呼吸暂停低通气综合征合并冠心病患者95例作为研究对象,根据治疗方法不同将其分为试验组(48例)与对照组(47例)。试验组使用Twin-block阻鼾器治疗,对照组使用Silensor阻鼾器治疗。观察两组呼吸紊乱指数、低通气指数、呼吸暂停指数、血氧饱和度、上气道截面积和肿瘤坏死因子水平。结果治疗后3个月,两组呼吸紊乱指数、低通气指数、呼吸暂停指数和肿瘤坏死因子水平均降低,且试验组低于对照组(P<0.05);治疗后3个月,两组血氧饱和度和上气道截面积均提高,且试验组高于对照组(P<0.05)。结论老年阻塞性睡眠呼吸暂停低通气综合征合并冠心病患者运用Twin-block阻鼾器治疗不仅有利于改善呼吸暂停、呼吸紊乱等症状,更有利于提升上气道横截面积与血氧饱和度,对改善缺血、缺氧的心肌细胞有重要作用,值得推广。Objective To investigate the application effect of Twin-block anti-snoring device in elderly patients with obstructive sleep apnea hypopnea syndrome complicated with coronary heart disease. Methods From January 2015 to December 2018, 95 elderly patients with obstructive sleep apnea hypopnea syndrome and coronary heart disease admitted in our hospital were selected as the research objects. According to different treatment methods, the patients were divided into experimental group(48 cases) and control group(47 cases). The experimental group was treated with Twin-block antisnoring device, and the control group was treated with Silensor anti-snoring device. The respiratory disturbance indexes,hypopnea indexes, apnea indexes, blood oxygen saturation, cross-sectional area of upper airway and tumor necrosis factor levels of the two groups were observed. Results At 3 months after treatment, the respiratory disturbance indexes, hypopnea indexes, apnea indexes and tumor necrosis factor levels in the two groups decreased, and those in the experimental group were lower than the control group(P<0.05);at 3 months after treatment, the blood oxygen saturation and the cross-sectional area of the upper airway in the two groups increased, and those in the experimental group were higher than the control group(P<0.05). Conclusion The use of Twin-block anti-snoring device in elderly patients with obstructive sleep apnea hypopnea syndrome complicated with coronary heart disease is not only beneficial to improve the symptoms of apnea and respiratory disorders, but also to improve the cross-sectional area of upper airway and blood oxygen saturation, it plays an important role in improving the myocardial cells of ischemia and hypoxia, which is worthy of popularization.
