基于聚类与关联规则的医院科室耗材使用模式研究

摘　　要：目的对比同病种耗材使用情况,发现病种-耗材使用模式;追踪单个病案的耗材使用清单,发现同病种耗材使用价值模式;基于以上模式,设计科室耗材使用评价方法。方法先对每个科室的耗材明细进行聚类,寻找病种-耗材之间的关联规则和考虑价值的病种-耗材的关联规则,设计耗材使用评分模型。结果对某三甲医院消化科半年的数据进行分析,将782个病案聚为4类,选取耗材费用和药品费用较高的51个病案进行计算,发现病种和耗材之间的关联规则以及病种和耗材之间带有价值系数的置信度。结论两类关联规则提供了参考依据,在此基础上设计耗材领用控制方法和评价模型。Objective Through comparing the usage of consumables of the same disease,to find the usage mode of the disease-consumables;through tracing the usage list of consumables of a single case,to find the usage value mode of consumables of the same disease;based on the model above,to design the evaluation method of consumables usage in departments.Methods Firstly,the detailed consumables of each department were clustered to find the association rules between diseases and consumables,and the association rules between diseases and consumables considering value.Based on the rules,the scoring model of consumables was designed.Results The data of Digestive Department in a tertiary A hospital for half a year were analyzed.782 medical records were grouped into4 categories.51 medical records with high cost of consumables and drugs were selected for calculation.The association rules between diseases and consumables,and the confidence degree with value coefficient between diseases and consumables were found.Conclusion Two kinds of association rules provides a reference basis.On this basis,the control method and evaluation model of consumables acquisition are designed.
