中药处方点评在提高临床医疗质量中的应用

摘　　要：目的通过对中药饮片的处方点评,促进医师处方书写的规范性,加强用药的合理性,进而提高医疗质量,保障医疗安全。方法随机抽取2016年10月~2017年9月我院的1200张(每月抽取100张)中药饮片处方进行点评,并以典型案例为例对点评策略进行解析。结果1200张中药饮片处方中,不合理处方为139张,占处方总量的11.58%。其中,处方的不合理率逐月下降,由2016年10月的18.00%下降为2017年9月的3.00%,其中临床诊断、医师签章规范、特殊煎法、单味药超剂量等不规范问题均逐渐减少。结论通过每月定期门诊处方点评,本院处方的规范性及合理率均得到显著提高,从而保证了患者的用药安全,减少了医疗隐患,提高了医疗质量,取得了良好效果。Objective To promote the standardization of doctors′prescription writing and strengthen the rationality of drug use by reviewing the prescriptions of Chinese herbal pieces,so as to improve the medical quality and ensure the medical safety.Methods A total of 1200 prescriptions(100 prescriptions per month)of Chinese herbal pieces were randomly selected from October 2016 to September 2017 for comment,and typical cases were taken as examples to analyze the comment strategy.Results Of the 1200 prescriptions for Chinese herbal pieces,139 were unreasonable,accounting for 11.58%of the total.Among them,the irrational rate of prescriptions decreased month by month,from 18.00%in October 2016 to 3.00%in September 2017.Among them,non-standard problems such as clinical diagnosis,doctors′signature standard,special decoction method,and single drug overdose gradually decreased.Conclusion Through regular monthly outpatient prescription reviews,the standardization and rationality of prescriptions is improving significantly,ensuring the medication safety of patients,reducing the occurrence of medical risks,improving the medical quality significantly and achieving good results.
