通窍化栓汤联合康复推拿对脑梗死患者神经功能恢复情况、ET-1和NO水平的影响

摘　　要：目的:探讨通窍化栓汤联合康复推拿对脑梗死患者神经功能恢复情况、内皮素1(ET-1)和一氧化氮(NO)水平的影响。方法:选取2017年6月—2019年6月符合纳入标准在我院神经内科就诊的80例脑梗死患者,随机单盲阳性药物对照试验法将其分为对照组(40例)和观察组(40例)。两组均予以常规对症及康复推拿治疗,观察组则在此基础上加用通窍化栓汤。两组均连续治疗28 d。观察并比较两组治疗前后的同型半胱氨酸(Hcy)、ET-1、NO和炎症因子水平、美国国立卫生研究院卒中量表(NIHSS)评分、中医证候积分及临床疗效。结果:治疗后,两组血清Hcy、ET-1、高敏C反应蛋白(hs-CRP)、肿瘤坏死因子α(TNF-α)及白细胞介素8(IL-8)水平、NIHSS评分以及中医证候积分均较治疗前显著降低,且观察组低于对照组(P<0.05);两组NO水平较治疗前显著升高,且观察组高于对照组(P<0.05)。观察组总有效率为90%(36/40),显著高于对照组的72.25%(29/40)(P<0.05)。结论:通窍化栓汤联合康复推拿可控制脑梗死患者的炎症反应、抑制血管内皮功能损伤、改善脑循环,有效恢复其神经功能、缓解临床症状,疗效显著。Objective:To explore the effect of Tongqiao Huashuan Decoction combined with rehabilitative tuina on neurological function recovery,the levels of endothelin-1(ET-1)and nitric oxide(NO)in patients with cerebral infarction.Methods:80 patients met the inclusion criteria were randomly divided into the control group(n=40)and the observation group(n=40)by single blind positive drug control test.The control group was treated with conventional treatment and rehabilitative tuina;on which basis,the observation group was also treated with Tongqiao Huashuan Decoction.Both groups were treated continuously for 28 days.The levels of homocysteine(Hcy),ET-1,NO and inflammatory factors,NIHSS score and TCM symptom score were observed and compared before and after the treatment in the two groups.The clinical efficacy was also compared between the two groups.Results:The levels of serum Hcy,ET-1,high sensitive c-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and interleukin-8(IL-8),NIHSS score and TCM symptom score were significantly decreased in the two groups after the treatment than those before the treatment(P<0.05);of which the decreases in the observation group were more significant(P<0.05).The level of NO was significantly increased in the two groups after the treatment than that before the treatment(P<0.05);the increase in the observation group was more significant(P<0.05).The total effective rate of the observation group was 90.00%(36/40),which was significantly higher than 72.25%(29/40)of the control group(P<0.05).Conclusion:Tongqiao Huashuan Decoction combined with rehabilitative tuina can inhibit the inflammatory reaction of cerebral infarction and vascular endothelial function damage,it also can improve cerebral circulation and neurological function,as well as relieve clinical symptoms.The therapy has significant curative effect.
