右美托咪定联合罗哌卡因在超声引导下肌间沟臂丛麻醉中的应用

摘　　要：目的探讨右美托咪定联合罗哌卡因在超声引导下肌间沟臂丛麻醉中的临床应用价值。方法选择2017年2月~2019年3月本院收治的于臂丛神经阻滞下完成的上肢相关手术治疗者80例,按照随机数字表法分为两组,每组40例。对照组患者实施超声引导下臂丛神经阻滞,选择肌间沟穿刺麻醉,使用剂量为0.5%罗哌卡因23 mL,明确定位后给予单次肌间沟注射,观察组则联合使用右美托咪定,比较两组麻醉效果中感觉及运动神经阻滞例数,统计术后住院时间及两组围麻醉期间发生的相关并发症。结果观察组感觉神经阻滞例数多于对照组(P<0.05),运动神经阻滞例数多于对照组(P<0.05)。观察组出现高血压、心动过缓、心动过速、呼吸抑制的总发生率明显低于对照组(P<0.05),观察组术后住院时间短于对照组(P<0.05)。结论对于超声引导下臂丛神经阻滞麻醉者联合使用右美托咪定,可有效减少麻醉相关并发症,确保麻醉效果,缩短住院时间。Objective To investigate the clinical value of dexmedetomidine combined with ropivacaine in ultrasound-guided intermuscular groove brachial plexus anesthesia.Methods Eighty patients with upper extremity-related surgery performed under brachial plexus block from February 2017 to March 2019 were enrolled in the hospital.They were divided into two groups according to random number method,with 40 cases in each group.The patients in the control group underwent ultrasound-guided brachial plexus block,and selected the intermuscular sulcus anesthesia.The dose was 0.5%ropivacaine 23 mL,and the single intermuscular groove was given after the localization.The observation group was combined with the dexmedetomidine.The number of sensory and motor block in the anesthesia effect of the two groups was compared.The postoperative hospital stay and related complications during the anesthesia between the two groups were counted.Results The number of sensory nerve block in the observation group was more than that in the control group(P<0.05),and the number of motor block was more than that in the control group(P<0.05).The overall incidence of hypertension,bradycardia,tachycardia and respiratory depression in the observation group was significantly lower than that in the control group(P<0.05).The postoperative hospital stay was shorter in the observation group than that in the control group(P<0.05).Conclusion The combination of dexmedetomidine in patients with brachial plexus block under ultrasound guidance can effectively reduce anesthesia-related complications,ensure anesthesia and shorten hospital stay.
