宫腔镜联合经阴道彩超诊断子宫内膜病变的价值研究

摘　　要：目的探讨宫腔镜联合经阴道彩超诊断子宫内膜病变的价值。方法选取2017年2月~2019年2月间本院收治的79例疑似子宫内膜病变患者为研究对象,所有患者均给予HS及TVS检查,并取子宫内膜活检,以子宫内膜活检病理结果为诊断标准,比较观察组(HS联合TVS)、对照组A(HS)、对照组B(TVS)的诊断效果。结果79例患者的活检病理诊断结果为:子宫内膜癌6例(7.59%)、黏膜下子宫肌瘤14例(17.72%)、子宫内膜增生17例(21.52%)、子宫内膜息肉22例(27.85%)、子宫内膜炎12例(15.19%)、正常子宫内膜8例(10.13%)。与对照组A及对照组B单独诊断相比,观察组诊断子宫内膜疾病的准确率、敏感度、特异度均为最高;对照组A效能较对照组B高;三组在检测EC及黏膜下子宫肌瘤两种疾病准确率方面无显著差异(P>0.05);在检测EH、EP、子宫内膜炎及正常子宫内膜方面,观察组与对照组A、B相比准确率高,有显著差异(P<0.05),对照组A与对照组B相比准确率高,有显著差异(P<0.05)。结论HS联合TVS诊断可提高子宫内膜病变诊断的准确率,有利于疾病的鉴别诊断及治疗方案的选择,有较好的临床应用价值。Objective To explore the value of hysteroscopy combined with transvaginal ultrasound in the diagnosis of endometrial lesions.Methods A total of 79 patients with suspected endometrial disease admitted in the First Hospital of Fuzhou City between February 2017 and February 2019 were selected as the study subjects.All patients were given hysteroscopy(HS)and transvaginal ultrasound(TVS)examination,and biopsies were performed at endometrium.The pathological results of endometrial biopsy were taken as the diagnostic criteria.The diagnostic effects of the observation group(HS combined TVS),the control group A(HS),and the control group B(TVS)were compared.Results The pathological diagnosis results of 79 cases were:6 cases of endometrial carcinoma(7.59%),14 cases of submucous myoma(17.72%),17 cases of endometrial hyperplasia(21.52%),22 cases of endometrial polyp(27.85%),12 cases of endometritis(15.19%),8 cases of normal endometrium(10.13%).Compared with control group A and control group B,the accuracy,sensitivity and specificity of endometrial disease diagnosis in the observation group were the highest;the efficacy of control group A was higher than that of control group B;there was no significant difference in the accuracy of EC and submucosal leiomyoma detection between the three groups(P>0.05);the detection rate of the observation group was higher than that of control group A and B in detecting EH,EP,endometritis and normal endometrium,and the accuracy of the control group A was higher than that of the control group B(P<0.05).Conclusion HS combined with TVS can improve the accuracy of diagnosis of endometrial lesions,is conducive to the differential diagnosis of diseases and the choosing of appropriate treatment options,thus being of good clinical application value.
