摘　　要：慢性鼻-鼻窦炎伴鼻息肉(Chronic rhinosinusitis with nasal polyps,CRSw NP)是耳鼻喉科的常见病和多发病,近年来其发病率呈上升趋势,从住院患者可以发现其手术率也很高,经过流行病学大规模统计得出不同人种、不同地区的总发病率约为1%~4%。由于功能性鼻内镜技术近年来发展速度很快,因此对于此类患者多通过手术治疗,能得到很好的效果,不仅缓解了患者的痛苦,而且他们的生活质量也大大提高。鼻内镜手术近年来大量开展,仪器设备越来越智能化,临床医师对于鼻内镜的操作也越来越成熟,但通过临床大数据反馈,其手术后复发率仍然较高,很多患者已经进行了多次手术治疗,给患者造成痛苦的同时也带来了经济上的压力。虽然经过了大量的研究,但其机制仍然没有明确,除了鼻内镜手术治疗外最主要的还是对糖皮质激素的应用,近几年鼻喷激素大量流行,相比较全身应用激素,它有副作用小、能直达病灶等优点。怎样做到靶向和精准治疗,我们还有很长的路要走,对其发病机制的研究仍然是我们的方向。此文就IL-6与各因子之间的关系进行了总结,对于下一步靶向治疗鼻息肉有了新的观点。Chronic rhinosinusitis with nasal polyps(crsw NP)is a common and frequently occurring disease in otorhinolaryngology.In recent years,the incidence of crsw NP has been on the rise.It can be found that the operation rate of crsw NP is also very high from the inpatients.According to the epidemiological model,the total incidence of crsw NP in different ethnic groups and regions is about 1%-4%.Because of the rapid development of functional endoscopic technology in recent years,it can get good results for such patients through surgical treatment,not only to alleviate the pain of patients,but also to improve their quality of life.In recent years,a large number of endoscopic surgery has been carried out,with more and more intelligent instruments and equipment.Clinicians are also more and more mature in the operation of endoscopic surgery.However,through the feedback of clinical big data,the recurrence rate after surgery is still high.Many patients have been operated for many times,causing pain to patients,but also bringing economic pressure.Although a lot of research has been done,its mechanism is still not clear.In addition to endoscopic surgery,the most important thing is the application of glucocorticoids.In recent years,a large number of nasal spray hormones are popular.Compared with systemic application of hormones,it has the advantages of small side effects and direct access to the focus.How to achieve targeted and precise treatment,we still have a long way to go,the study of its pathogenesis is still our direction.This paper summarizes the relationship between IL-6 and various factors,and has a new point of view for the next targeted treatment of nasal polyps.
