摘 要:目的探讨代谢综合征(MS)相关指标对血清前列腺特异性抗原(PSA)、前列腺特异性抗原密度(PSAD)和前列腺体积(PV)的影响。方法收集2011年8月-2017年7月西安交通大学第一附属医院体检中心进行健康查体的4 404例60~95岁男性资料,询问病史,通过直肠指检、超声排除前列腺癌,测量人群收缩压(SBP)、舒张压(DBP)、体重(W)、身高(H)、腰围(WC)、体重指数(BMI)。生化分析包括晨空腹血浆葡萄糖(FPG)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、血清PSA、尿常规分析等。结果 MS相关指标SBP、TG、HDL-C、LDL-C与血清PSA呈正相关(r=0.084、0.065、0.111和0.080,P<0.05),DBP、TC与血清PSA呈负相关(r=-0.079和-0.083,P<0.05);FPG、LDL-C与PV呈正相关(r=0.112和0.033,P<0.05);BMI、SBP、TG、HDL-C、LDL-C与PSAD呈正相关(r=0.039、0.087、0.059、0.098和0.073,P<0.05),WC、DBP、FPG、TC与PSAD呈负相关(r=-0.039、-0.081、-0.063和-0.074,P<0.05)。进一步行多元线性回归分析发现,MS影响血清PSA、PSAD水平的主要因素为SBP、DBP和HDL(P<0.05),影响PV水平的主要因素为FPG(P<0.05)。结论老年MS患者血清PSA、PSAD水平下降,PV增加。其中血压变化与血清PSA水平密切相关,血糖变化与PV相关。Objective To study the effect of the related parameters of the metabolic syndrome on serum prostate specific antigen(PSA), prostate specific antigen density(PSAD) and the prostate volume(PV). Methods A total of 4,404 men aged 60 to 95 years were enrolled from August 2011 to July 2017 in the First Affiliated Hospital of Xi’an Jiaotong University. The histories of patients combined with digital rectal examination and ultrasound were used to exclude prostate cancer. Systolic blood pressure(SBP), diastolic blood pressure(DBP), weight(W), height(H), waist circumference(WC) and body mass index(BMI) were measured. Biochemical analyses including fasting plasma glucose(FPG), total cholesterol(TC), triglyceride(TG), high-density lipoprotein cholesterol(HDL-C), lowdensity lipoprotein cholesterol(LDL-C), serum PSA and urine routine analysis were detected. Results The liner regression analysis showed the SBP, TG, HDL-C and LDL-C had statistically positive correlations with serum PSA(P<0.05), while DBP and CHOL had statistically negative correlations with serum PSA(P<0.05). And FPG, LDL-C had statistically positive correlations with PV(P<0.05). BMI, SBP, TG, HDL-C and LDL-C had statistically positive correlations with serum PSAD(P<0.05), and WC, DBP, FPG, TC and PSAD had statistically negative correlations with PSAD(P<0.05). After multiple linear regression analysis, it showed that PSA, PSAD levels are statistical association with SBP, DBP and HDL(P<0.05), while PV was statistical association with FPG(P<0.05). Conclusions The levels of serum PSA and PSAD decrease and PV increases in elderly MS patients. The changes of BP might relate to serum PSA levels, while DM might connect with PV.
