PDCA循环管理在ICU导尿管相关性尿路感染控制中的应用

摘　　要：目的探讨PDCA循环管理模式在降低ICU导尿管相关性尿路感染(CAUTI)发生率中的应用效果。方法将2017年1-12月我院ICU留置导尿管的647例患者作为对照组,采用常规管理。对发生CAUTI患者查找发生CAUTI的真因,制定整改措施;将2018年1-12月在我院ICU留置导尿管的559例患者作为观察组,采用PDCA循环管理模式进行管理,比较两组患者CAUTI发生率。结果观察组患者留置尿管率、CAUTI发生率低于对照组(P <0. 05);CAUTI的防控措施执行率中,尿管留置时间≥7 d率低于对照组(P <0. 05),尿道口清洁率、尿袋固定正确率、无菌操作合格率均高于对照组(P <0. 05)。结论 PDCA循环管理能显著提高ICU导尿管相关性尿路CAUTI的防控措施落实率,有效降低CAUTI发生率。Objective To explore the effect of PDCA circulation management mode in reducing the incidence of ICU catheter-associated urinary tract infection control( CAUTI). Methods We selected 647 cases of patients with ICU indwelling catheters in our hospital from January to December 2017,and they were used as the control group and was given routinely administered to find out the true cause of CAUTI in patients with CAUTI and formulate corrective measures. Meanwhile we taken the 559 cases of patients who placed the catheter in the ICU in our hospital from January to December 2018 as the observation group and use the PDCA cycle management mode to manage and compare the incidence of patients with CAUTI between the two groups. Results The rate of indwelling catheter and the incidence of CAUTI in the observation group were lower than those in the control group( P < 0. 05). Among the implementation rates of the prevention and control measures of CAUTI,the rate of indwelling catheter time ≥7 d was lower than that in the control group( P <0. 05). The urethral cleansing rate,the accuracy rate of urinary bag fixation,and the aseptic operation qualification rate in the observation group were higher than those of the control group( P < 0. 05). Conclusion The application of PDCA cycle management could significantly improve the implementation rate of ICU CAUTI prevention and control measures,and effectively reduce the incidence of CAUTI.
