摘 要:目的研究早产儿脐血清白细胞介素-6(IL-6)和IL-10对产妇绒毛膜羊膜炎的诊断价值,并探讨其与早产儿脑损伤的相关性。方法选取2014年6月至2018年6月在该院出生的胎龄小于或等于34周早产儿(胎龄)共138例。根据产妇是否存在绒毛膜羊膜炎,分为病例组(65例)和对照组(73例)。所有患儿出生后抽脐血行IL-6、IL-10检测,纠正胎龄40周时行颅脑磁共振检查,观察对比两组患儿脑损伤的发生情况,并根据是否存在脑损伤,将所有病例分为脑损伤组(28例)、非脑损伤组(110例),对比两组患儿中IL-6和IL-10水平。结果病例组患儿脐血清IL-6水平显著高于对照组,脐血清IL-10水平显著低于对照组,差异均有统计学意义(P<0.05)。脑损伤组患儿脐血IL-6水平显著高于非脑损伤组,脐血IL-10水平显著低于非脑损伤组,差异均有统计学意义(P<0.05)。结论早产儿脐血清IL-6、IL-10水平是预测绒毛膜羊膜炎的有用指标,且对脑损伤的诊断有一定预测价值。Objective To investigate the diagnostic value of interleukin-6(IL-6)and IL-10 in premature infants with umbilical cord blood innocence on maternal chorionicamnionitis,and to explore its correlation with brain injury in premature infants.Methods A total of 138 premature infants with gestational age less than or equal to 34 weeks(gestational age)born in the hospital from June 2014 to June 2018 were selected.According to the presence or absence of chorionicamnionitis,the patients were divided into case group(65 cases)and control group(73 cases).After birth,umbilical cord blood was extracted to detect IL-6 and IL-10,and craniocerebral magnetic resonance imaging was performed at 40 weeks of gestational age.The occurrence of brain injury in the two groups was observed and compared.According to the presence or absence of brain injury,all cases were divided into the brain injury group(28 cases)and the non-brain injury group(110 cases),and the levels of IL-6 and IL-10 in the two groups were compared.Results The level of IL-6 in the case group was significantly higher than that in the control group,and the level of IL-10 in the case group was significantly lower than that in the control group,with statistically significant differences(P<0.05).The level of IL-6 in umbilical cord blood in the brain injury group was significantly higher than that in the non-brain injury group,and the level of IL-10 in umbilical cord blood was significantly lower than that in the non-brain injury group,with statistically significant differences(P<0.05).Conclusion The levels of IL-6 and IL-10 in umbilical cord blood serum of premature infants are useful indicators for predicting chorionicamnionitis,and have certain predictive value for the diagnosis of brain injury.
