不同甲氨蝶呤应用方案联合米非司酮治疗异位妊娠的对照分析

摘　　要：目的对照分析不同甲氨蝶呤应用方案联合米非司酮治疗异位妊娠的临床效果。方法选取2018年6月~2019年5月中山市三乡医院妇产科54例异位妊娠患者进行研究,将其随机分成对照组和研究组,对照组(27例)甲氨蝶呤治疗方案为持续肌注,并联合米非司酮治疗,研究组(27例)甲氨蝶呤治疗方案为单次肌注,并联合米非司酮治疗,对照分析两组临床效果、治疗前后血清HCG(绒毛膜促性腺激素)水平、血清HCG恢复正常时间、住院时间、不良反应情况。结果从临床总有效率上来看,研究组为92.59%,对照组为70.37%,研究组高于对照组,差异有统计学意义(P<0.05);治疗前两组患者的血清HCG水平比较差异无统计学意义(P>0.05),治疗后4、7、10、14d两组血清HCG水平均下降,研究组均低于对照组,差异有统计学意义(P<0.05);研究组血清HCG恢复正常时间比对照组短,住院时间也比对照组短,差异有统计学意义(P<0.05);从不良反应发生率上来看,研究组为7.41%,对照组为29.63%,研究组低于对照组,差异有统计学意义(P<0.05)。结论针对异位妊娠患者来说,采用单次肌注甲氨蝶呤联合米非司酮治疗可以取得良好效果,可以有效控制血清HCG水平,促进其恢复,缩短住院时间,另外具有较高的安全性,临床价值显著,值得推广及应用。Objective To compare and analyze the clinical effects of different methotrexate regimens combined with mifepristone in the treatment of ectopic pregnancy.Methods 54 patients with ectopic pregnancy who were treated in the Department of Obstetrics and Gynecology of Sanxiang Hospital in Zhongshan City from June 2018 to May 2019 were selected and randomly divided into control group and study group.The control group(27 cases)received continuous intramuscular methotrexate treatment regimen in combination with mifepristone.The study group(27 cases)received single intramuscular methotrexate treatment regimen in combination with mifepristone.The clinical effects,serum HCG(chorionic gonadotropin)levels before and after treatment,time for serum HCG to return normal level,hospitalization time and adverse reactions were compared between the two groups.Results The total clinical effective rate was 92.59%in the study group and 70.37%in the control group.The study group was higher than the control group,and the difference was statistically significant(P<0.05).There was no significant difference in serum HCG levels between the two groups of patients before treatment(P>0.05).Serum HCG levels decreased in the two groups at 4,7,10 and 14 days after treatment,and the study group was lower than the control group,with statistically significant difference(P<0.05).The time for serum HCG to return normal level in the study group was shorter than that in the control group,and the hospitalization time was also shorter than that in the control group.The differences were statistically significant(P<0.05).The incidence of adverse reactions in the study group was 7.41%,and that in the control group was 29.63%.The study group was lower than the control group,and the difference was statistically significant(P<0.05).Conclusion For patients with ectopic pregnancy,single intramuscular injection of methotrexate combined with mifepristone can achieve good results,effectively control serum HCG levels,promote recovery,shorten hospitalization time,have h
