改良三腔硅胶尿管联合负压吸引护理膀胱血块填塞的应用效果

摘　　要：目的研究改良三腔硅胶尿管联合负压吸引护理膀胱血块填塞的应用效果。方法选取本院2013年1月~2018年10月诊治的膀胱血块填塞患者160例分为改良组与普通组,各80例,改良组采用改良三腔硅胶尿管联合负压吸引方法,普通组采用普通三腔硅胶尿管联合负压吸引方法。评估清除血块时间,比较两组患者血块清除量、血块大小、应用冲洗液量、血块清除成功率以及并发症。结果两组患者发生血块填塞的时间无统计学差异(P>0.05),但改良组彻底清除血块时间以及护理后患者尿色恢复正常的时间明显短于普通组(P<0.05);改良组患者清除血块大小以及血块量明显高于普通组,冲洗液用量明显少于普通组(P<0.05);改良组血块清除成功率显著高于普通组(P<0.05),尿路感染、狭窄等并发症发生率与普通组比较差异无统计学意义(P>0.05)。结论改良三腔硅胶尿管联合负压吸引护理应用于膀胱血块填塞可有效提高患者膀胱内部血块清除率,缩短清除时间,减少冲洗液用量以及并发症。Objective To investigate the application effect of modified three-chamber silicone catheter combined with negative pressure suction for nursing bladder blood clot tamponade.Methods 160 patients with bladder blood clot tamponade treated in our hospital from January 2013 to October 2018 were selected and divided into the modified group and the common group,with 80 patients in each group.The modified group was treated with modified three-chamber silicone catheter combined with negative pressure suction,and the common group was treated with common three-chamber silicone catheter combined with negative pressure suction.The time of blood clot removal was evaluated,and amount of blood clot removal,size of blood clots,volume of irrigation fluid,successful rate of blood clot removal and complications were compared between the two groups.Results There was no significant difference in the time to blood clot tamponade between the two groups(P>0.05).However,the time of complete blood clot removal and the time of urine color recovery in the modified group were significantly shorter than those in the common group(P<0.05).The size of blood clots and amount of blood clots were significantly higher than those of the common group,and volume of irrigation fluid was significantly lower than that of the common group(P<0.05).The success rate of blood clot removal in the modified group was significantly higher than that in the common group(P<0.05),and the incidence of complications such as urinary tract infections and stenosis was not significantly different from that in the common group(P>0.05).Conclusion Modified three-chamber silicone catheter combined with negative pressure suction nursing applied to bladder blood clot tamponade can effectively improve the blood clot clearance rate inside the bladder,shorten the removal time,reduce the amount of irrigation fluid and complications.
