摘　　要：目的观察连续皮内缝合法在心脏起搏器更换术中的应用效果。方法选取2016年1月至2019年4月在江苏大学附属医院心内科因原有起搏器电池耗竭行永久性心脏起搏器更换术的70例患者,分成皮内缝合组(37例,使用连续皮内缝合法缝合皮肤)及传统缝合组(33例,使用传统间断缝合法缝合皮肤)。比较两组间切口愈合情况,切口缝合时间、住院时间、术后囊袋血肿以及术后3个月随访情况。结果所有患者均Ⅰ期甲级愈合;两组手术缝合时间比较,差异无统计学意义(P>0.05)。与传统缝合组比较,皮内缝合组住院时间明显缩短,术后囊袋血肿发生率降低,差异有统计学意义(P<0.05)。术后3个月随访,皮内缝合组未见皮肤线头外露,传统缝合组2例外露。结论采用连续皮内缝合法缝合心脏起搏器更换术患者切口,可明显缩短患者住院时间,降低术后囊袋血肿的发生率,无需拆线,适合临床广泛应用。Objective To observe the effect of continuous intradermal suture in cardiac pacemaker replacement.Methods A total of 70 patients for permanent cardiac pacemaker replacement due to battery depletion of original pacemaker in Affiliated Hospital of Jiangsu University from January 2016 to April 2019 were selected and randomly divided into two groups.One group used continuous intradermal suture(37 cases)and the other group used traditional intermittent suture(33 cases).The incision healing,hospitalization time,surgical suture time,postoperative cystic hematoma and 3-month post-operative follow-up were compared between the two groups.Results All patients received class I healing.There was no significant difference in surgical suture time between the two groups(P>0.05).Compared with the traditional suture group,the hospitalization time in the intradermal suture group was shorter,and the incidence of postoperative cystic hematoma was lower(P<0.05).After 3-month follow-up,no skin line was exposed in the intradermal suture group,2 cases were exposed in the traditional suture group.Conclusion The continuous intradermal suture was used to treat the incision of cardiac pacemaker replacement,which significantly shortened the patient's hospitalization time,reduced the incidence of postoperative cystic hematoma,and did not need to remove the thread.It was suitable for clinical promotion.
