摘　　要：祖国医学认为,脾主运化、主四肢肌肉。"脾主肌肉"是以脾所运化的水谷精微充养全身肌肉;脾气健运,则肌肉丰盈强健;脾失健运,则肌肉萎软无力。现代医学认为线粒体是细胞中重要的细胞器,主要功能是进行氧化反应为细胞提供能量。慢性阻塞性肺疾病合并膈肌疲劳主要表现为呼吸肌无力萎缩,而呼吸运动所需能量源于线粒体的氧化反应。该文结合中医理论"脾主肌肉"对慢阻肺合并膈肌疲劳与线粒体能量代谢的关系进行探讨,冀以更深入地理解祖国医学中所述"脾主肌肉"的概念,同时为临床治疗慢阻肺合并膈肌疲劳提供思路。According to Traditional Chinese Medicine, the spleen governs transportation, blood and limb muscles, and the spleen is the "source of biochemistry of Qi and blood" and the "foundation of acquired nature". "Spleen governs muscle" is based on the function of governing transport as a physiological basis. Spleen as the source of Qi and blood biochemistry and transported the grain essence to nourish the whole body muscle.If spleen is healthy, muscle is rich and strong;spleen is not healthy, muscle atrophy and weakness. Modern medicine holds that mitochondria are important organelles in cells. Their main function is to provide energy for the metabolism of various substances in cells through oxidative reaction. Chronic obstructive pulmonary disease(COPD) with diaphragmatic fatigue is mainly manifested by respiratory muscle weakness atrophy and dyspnea. The energy required for human respiratory movement comes from the oxidative reaction of mitochondria in cells. Therefore, the energy required for diaphragm metabolic activity is closely related to mitochondria. This article will summarize the relationship between COPD with diaphragmatic fatigue and mitochondrial energy metabolism, in order to understand the concept of "Spleen governs muscle" mentioned in traditional Chinese medicine more deeply and provide treatment ideas for COPD patients with diaphragmatic fatigue.
