摘  要:目的分析亚硝酸盐(NIT)、白细胞酯酶(LE)、细菌计数(BACT)联合检测在尿路感染(UTI)筛查中的临床价值。方法选择2017年8月-2019年6月就诊于我院的疑为UTI患者84例。采集入选者尿液标本,采用Lab UMat2+Urised2全自动尿液分析工作站进行NIT、LE、BACT检测。观察NIT、LE、BACT检测结果,并以细菌培养结果作为金标准,计算NIT、LE、BACT诊断阴性预测值、阳性预测值、敏感度、特异度。结果84例疑为UTI患者中经细菌培养证实阳性32例(38.10%);LE+BACT、NIT+LE+BACT诊断敏感度(最高且均为90.63%)高于NIT;NIT诊断特异度(最高96.15%)高于LE、BACT、LE+BACT;NIT+LE+BACT阴性预测值(最高94.23%)高于NIT;NIT+LE+BACT阳性预测值(最高90.63%)高于LE、BACT、LE+BACT,差异有统计学意义(P<0.05)。结论NIT、LE、BACT联合检查有助于提高UTI诊断效能,有助于检出UTI,进而为UTI诊治提供指导,但UTI最终确诊需依据细菌培养结果。Objective To analyze the clinical value of combined detection of nitrite(NIT),leukocyte esterase(LE)and bacterial count(BACT)in urinary tract infection(UTI)screening.Methods Eighty-six patients with suspected UTI were enrolled in our hospital from August to September 20-19.Urine specimens were collected from the selected subjects,and NIT,LE,and BACT were detected using the Lab UMat2+Urised2 automatic urine analysis workstation.The results of NIT,LE and BACT were observed,and the results of bacterial culture were used as the gold standard to calculate the negative predictive value,positive predictive value,sensitivity and specificity of NIT,LE and BACT.Results Of the 84 patients with suspected UTI,32(3&10%)were confirmed by bacterial culture.The diagnostic sensitivity of LE+BACT and NIT+LE+BACT(highest and 90.63%)was higher than NIT;NIT diagnostic specificity(maximum 96.15%)higher than LE,BACT,LE+BACT;NIT+LE+BACT negative predictive value(up to 94.23%)is higher than NIT;NIT+LE+BACT positive predictive value(highest 90.63%)is higher than LE,BACT,LE+BACT,the difference was statistically significant(P<0.05).Conclusion The combined examination of NIT,LE and BACT can improve the diagnostic efficiency of UTI,help to detect UTI,and provide guidance for the diagnosis and treatment of UTI.However,the final diagnosis of UTI is based on bacterial culture results.
