摘　　要：介绍李赛美教授自拟加味五苓散的组方思路及其临床应用经验。《伤寒论》中的五苓散(由猪苓、茯苓、泽泻、桂枝、白术共5味中药组成)是治疗小便不利的著名方剂。李赛美教授根据多年临床经验,在五苓散的基础上,以薏苡仁代替猪苓,并加乌药、黄芪、熟附子,即自拟加味五苓散,加强了补气健脾及温阳利水之功,用于治疗各种泌尿系统疾病引起的小便不利且证属下焦虚寒、水湿内停的蓄水证患者,疗效显著。李赛美教授自拟加味五苓散的组方思路及其应用可为临床治疗以小便不利为主诉且属虚寒型蓄水证者提供辨治参考。The prescription thoughts and clinical application of modified Wuling Powder used by Professor LI SaiMei were summarized.Originated from Treatise on Febrile Diseases,Wuling Powder is a well-known prescription used to treat difficult urination,and is composed of Polyporus,Poria,Rhizoma Alismatis,Ramulus Cinnamomi and Rhizoma Atractylodis Macrocephalae.Based on years of clinical experience,Professor LI Sai-Mei modified the classic Wuling Powder by using Semen Coicis replacing Polyporus,and by adding Radix Linderae,Radix Astragali,and Radix Aconiti Lateralis Preparata.The modified Wuling Powder enhanced the actions of replenishing qi and invigorating spleen,and warming yang to induce diuresis,which can be used to treat difficult urination caused by various urinary system diseases and differentiated as deficiency cold in the lower energizer and interior retention of water-dampness.The modified Wuling Powder has achieved significant curative effect,and the prescription thoughts and clinical application will supply reference for the treatment of deficiency-cold fluid retention patients with the chief complaint of difficult urination.
