针刺治疗溃疡病穿孔与血管活性肠肽变化的相关性研究

摘　　要：【目的】探讨针刺疗法对溃疡病穿孔患者血清血管活性肠肽(VIP)、白细胞数、中性粒细胞百分比、降钙素原(PCT)的影响。【方法】选取60例溃疡病穿孔患者,在常规处理基础上,给予电针双侧足三里、中脘穴治疗,采用疏密波刺激,每次持续30 min,每2 h治疗1次,共治疗4次。观察患者治疗前后血清VIP、白细胞数、中性粒细胞百分比和PCT的变化情况。【结果】针刺治疗后,患者白细胞数、中性粒细胞百分比及PCT水平均较治疗前明显下降,血清VIP水平较治疗前明显升高,差异均有统计学意义(P<0.05)。【结论】电针足三里、中脘穴,可明显提高患者血清VIP水平,从而促进患者胃肠黏膜的修复、加快胃肠穿孔的愈合,并可促进患者胃肠功能恢复和控制炎症反应。Objective To investigate the effect of acupuncture on serum vasoactive intestinal peptide(VIP),white blood cell(WBC)count,neutrophil percentage,and procalcitonin(PCT)level in the patients with ulcerative perforation.Methods Based on the conventional treatment,60 ulcerative perforation patients were treated with dilatational wave at bilateral acupoints of Zusanli(ST36)and Zhongwan(CV12)for 30 minutes each time,once every 2 hours,in a total of four times.The changes of serum VIP,WBC count,neutrophil percentage,and PCT level in the patients were monitored before and after treatment.Results After acupuncture,WBC count,neutrophil percentage,and PCT level in the patients were obviously decreased,and serum VIP level was markedly increased,the difference being statistically significant(P<0.05 compared with those before treatment).Conclusion Electroacupuncture at Zusanli(ST36)and Zhongwan(CV12)acupoints can significantly increase serum VIP level,promote the repair of gastrointestinal mucosa,accelerate the healing of gastrointestinal perforation,promote the recovery of gastrointestinal function and relieve the inflammatory reaction.
